摘要
目的利用磁共振T2弛豫时间图成像(T2-mapping)技术,通过评估软骨的生理结构和生化成分的变化,可以在软骨形态发生变化之前诊断出早期的软骨损伤,并且可以定量监测软骨病变的进展,从而可以更加早期、准确地诊断关节软骨损伤。方法收集临床已确诊为不同程度膝关节骨性关节病(OA)的20例患者为实验组;另外筛选20例健康志愿者为对照组,MRI检查前3h,所有受试者均无剧烈运动,使用Philips 3.0T磁共振扫描仪,选择膝关节表面线圈,首先执行常规序列扫描,然后执行轴位T2-mapping序列扫描,将得到的软骨图像数据在工作站进行图像后处理,获得关节软骨的T2-mapping伪彩图像,分别测量膝关节的股骨内侧髁、外侧髁、股骨滑车区及髌骨这4个区域的软骨T2值,然后对数据进行统计学分析,分别测定实验组和对照组的T2平均值,采用t检验比较两组数据的差异,若P<0.05说明差异具有统计学意义。结果实验组关节软骨T2平均值为(63.41±12.53)ms,对照组关节软骨T2平均值为(41.53±6.45)ms,实验组关节软骨T2平均值明显高于对照组,两组间差异具有统计学意义。结论T2-mapping图像中关节软骨的T2值与游离水含量呈显著正相关。因此,关节软骨的损伤越严重,T2值就越高,并且可以在关节软骨发生形态变化之前检测到软骨的生化结构变化,从而提供早期的临床治疗证据,有助于确定合理的治疗方案,可以定量监测软骨病变的进展,具有很高的临床应用价值。
Objective Using magnetic resonance T2 relaxation time mapping imaging(T2-mapping)technology,by evaluating changes in cartilage physiological structure and biochemical components,it can diagnose early cartilage damage before morphological changes occur,and quantitatively monitor the progress of cartilage lesions.Make the diagnosis of articular cartilage damage by magnetic resonance imaging more early and accurate.Methods It is planned to collect 20 patients who have been clinically diagnosed with various degrees of knee osteoarthropathy(OA)in our hospital as the experimental group;in addition,20 healthy volunteers will be selected as the control group,and all subjects should be within 3 hours before the MRI examination No strenuous exercise.Use Philips 3.0 T magnetic resonance scanner,select knee joint surface coils,perform conventional sequence scanning first,and then perform axial T2-mapping sequence scanning,and then post-process the obtained cartilage image data on the workstation to obtain the T2-of the articular cartilage.Mapping pseudo-color images,measure the cartilage T2 values of the medial femoral condyle,lateral condyle,femoral trochlear area,and patella of the knee joint,and then perform statistical analysis on the data to determine the average T2 values of the experimental group and the control group,respectively,Using t test to compare the difference between the two groups of data,P<0.05 indicates that the difference is statistically significant.Results The average T2 of articular cartilage in the experimental group was(63.41±12.53)ms,and the average T2 of articular cartilage in the control group was(41.53±6.45)ms.The T2 value of articular cartilage in the experimental group was significantly higher than that of the control group.The difference between the two groups was statistically significant.Conclusions The T2 value of articular cartilage in the T2-mapping image is significantly positively correlated with free water content.Therefore,the heavier the articular cartilage damage,the higher the T2 value,which can detect cartilage structural changes before the morphological changes of the articular cartilage,providing early clinical treatment The diagnostic basis of this method is conducive to determining a reasonable treatment plan,and can quantitatively monitor the progress of cartilage lesions,which has high clinical application value.
