2019-2020年某医院995份中药注射剂专项点评分析

Analysis of 995 Special Comments on Traditional Chinese Medicine Injections in A Hospital from 2019 to 2020

目的:分析医院中药注射剂使用情况,为临床合理用药提供对策。方法:选取2019年10月至2020年6月南昌市洪都中医院1603份住院患者病例作为研究对象。统计、计算中药注射剂在各科室中的使用情况,并对物使用情况进行专项点评,处方中包含丹红注射液、醒脑静注射液、红花黄色素氯化钠注射液等8种药品,统计中药注射剂不合理使用情况及不良反应发生情况。结果:1603份病历中使用中药注射剂的有995份,其中骨科50.45%,急诊科7.54%,康复科1.31%,内科27.44%,软伤科0.20%,肾内科1.20%,外科1.81%,针灸科9.05%,重症科1.01%;995份使用中药注射剂病历中,共有408例中药注射剂使用不合理,占比41.01%,其中联合用药不适宜占10.29%,溶媒选择不合理占43.14%,用法用量不适宜占24.02%,适应证不适宜占22.55%;408份中药注射剂使用不合理病历中,有55份病历出现不良反应,占比13.48%,其中全身性损害占5.45%,呼吸系统损害占1.82%,心血管系统损害占20.00%,皮肤及其附件损害占72.73%。结论:该医院中药注射剂不合理用药情况普遍存在,应严格控制其无指征滥用,加强专业药学知识培训,促进临床合理用药。

Objective:To analyze the use of traditional Chinese medicine(TCM)injection in hospital and provide countermeasures for clinical rational drug use.Methods:1603 inpatients in Nanchang Hongdu Hospital of Traditional Chinese Medicine from October 2019 to June 2020 were selected as the research object.Count and calculate the use of traditional Chinese medicine injections in various departments,and make special comments on the use of drugs.The prescription includes 8 drugs such as Danhong injection,Xingnaojing injection and safflower yellow sodium chloride injection.Count the unreasonable use of traditional Chinese medicine injections and the occurrence of adverse reactions.Results:995 of 1603 medical records,including 50.45%in orthopedics,emergency department 7.54%,rehabilitation section 1.31%,internal medicine 27.44%,soft injury section 0.20%,nephrology 1.20%,saurgery department 1.81%,acupuncture division 9.05%,critical department 1.01%;995 medical records of TCM injections,a total of 408 cases of TCM injections were unreasonable,for 41.01%,among them,the combination of medication is not suitable for 10.29%,improper choice of solvent medium is 43.14%,inappropriate usage for 24.02%,Inindications accounted for 22.55%;408 unreasonable medical records of TCM injections,there were 55 medical records with adverse reactions,for 13.48%,the total body damage accounted for 5.45%,respiratory system damage is 1.82%,cardiovascular system damage is 20.00%,skin and accessory damage is 72.73%.Conclusion:Unrational abuse of traditional Chinese medicine injection should be strictly controlled,strengthen professional pharmaceutical knowledge training,and promote rational clinical medication.
