肺部磨玻璃结节的高分辨率CT征象对肺结节良、恶性的鉴别诊断价值分析

The Value of High Resolution CT Signs of Ground Glass Nodules in the Differential Diagnosis of Benign and Malignant Pulmonary Nodules

目的:分析肺部磨玻璃结节的高分辨率CT征象对肺结节良、恶性的鉴别诊断价值。方法:将2018年1月~2020年4月在本院就诊的62例肺结节患者视为研究对象,患者高分辨率CT检查均具有肺部磨玻璃结节特征。根据其手术或者病理结果纳入良性组与恶性组(n=31)。比较患者的肺部磨玻璃结节高分辨率CT征象的特征区别,附近结构变化情况等。结果:良性组患者的分叶征、毛刺征及密度等与恶性组比较差异明显,有统计学意义(P<0.05)。在性状、大小及支气管征等方面比较,良性组与恶性组无显著区别(P>0.05)。分叶征诊断恶性肺部磨玻璃结节的敏感度是74.19%,其次是毛刺征、清除毛糙边界、密度及囊状透亮影等。结论:高分辨率CT征象可清晰呈现肺部磨玻璃结节的状态,有助于肺结核患者良、恶性判断。

Objective:To analyze the value of high resolution CT signs of pulmonary ground glass nodules in the differential diagnosis of benign and malignant pulmonary nodules.Methods:A total of 62 patients with pulmonary nodules admitted to our hospital from January 2018 to April 2020 were considered as subjects of the study.All the patients showed characteristics of ground glass pulmonary nodules in high-resolution CT examination.According to its surgical or pathological results,the benign group and the malignant group were included(n=31).To compare the characteristics of patients with high resolution CT signs of ground glass nodules,the changes of nearby structures,etc.Results:The difference of foliation,burr and density between the benign group and the malignant group was significant(P<0.05).In terms of traits,size,and bronchial signs,there was no significant difference between the benign group and the malignant group(P>0.05).The sensitivity of the lobe sign to diagnose malignant pulmonary ground glass nodules is 74.19%,followed by the burr sign,removing rough borders,density,and cystic translucent shadows.Conclusion:High resolution CT can clearly present the status of ground glass nodules in the lungs,which is helpful for the diagnosis of benign and malignant pulmonary tuberculosis patients.
