以瘀为核心辨治癌痛

Syndrome Differentiation and Treatment of Cancer Pain with Blood Stasis as the Core

癌痛的病理基础是风、火、瘀,瘀血阻滞为癌痛发作的核心病机。瘀血兼夹痰浊、气滞、寒凝等病理因素,壅遏日久,瘀而化火,化火生风,表现出不同的疼痛方式。瘀而化火多表现为针刺样疼痛或灼痛;化火生风主要表现为刺痛、灼痛、放电样疼痛、自发性疼痛、痛觉超敏、异常疼痛等。治疗时需要根据具体疼痛表现,给予活血、凉血、熄风之法,或单法应用,或合并运用,可单纯中药治疗,亦可联合西医三阶梯止痛药物使用。

The pathological basis of cancer pain is wind,fire and blood stasis.The obstruction of blood stasis is the core pathogenesis of cancer pain.Blood stasis is accompanied by pathological factors such as phlegm turbidity,qi stagnation and cold coagulation.It blocks the channel for a long time,transforms into fire,generates wind and shows different ways of pain.Blood stasis transforming into fire are mostly manifested as acupuncture like pain or burning pain.The main manifestations of transforming into fire and generating wind are tingling,burning,discharge like pain,spontaneous pain,hyperalgesia,abnormal pain,etc.According to the specific pain manifestations,the treatment needs to be given the methods of promoting blood circulation,cooling blood and quenching wind,or single method or combined application.It can be treated with Chinese materia medica alone or combined with three-step analgesic drugs of Western medicine.
