基于红外线热成像探讨精灸治疗膝骨关节炎的临床观察 被引量:5

摘要 目的基于红外线热成像探讨精灸治疗膝骨关节炎(KOA)的疗效。方法选取2020年7月至2021年2月在云浮市中医院针灸推拿科就诊的膝骨关节炎患者90例作为研究对象,使用随机数字表法分为治疗组与对照组,每组各45例。对照组予常规药物治疗;治疗组在对照组处理基础上加用精灸治疗,观察两组治疗疗效、治疗前后疼痛、西安大略和麦克马斯特大学骨关节炎调查量表(WOMAC)评分、五次坐立测试时间(FTSST)及红外热像图变化。结果治疗组治疗总有效率高于对照组(93.33%vs.73.33%),差异有统计学意义(P<0.05);两组治疗前视觉模拟评分法(VAS)评分、WOMAC总积分、FTSST时间比较差异无统计学意义(P>0.05);治疗后两组VAS评分、WOMAC总积分、FTSST时间低于治疗前,膝关节前侧温度高于治疗前,差异有统计学意义(P<0.05);且治疗组VAS评分、WOMAC总积分、FTSST时间低于对照组,而膝关节前侧温度高于对照组,差异有统计学意义(P<0.05)。结论精灸治疗后KOA患者疼痛、膝关节功能明显改善、FTSST时间缩短,且采用红外线热成像明显观察到患者膝关节温度变化,进一步反映了临床治疗效果,为精灸治疗KOA疗效提供客观依据。 Objective To explore the efficacy of pure moxibustion in the treatment of knee osteoarthritis based on infrared thermal imaging.Methods A total of 90 patients with knee osteoarthritis who treated in the Department of Acupuncture of Moxibustion and Massage of Yunfu City Hospital of Traditional Chinese Medicine from July 2020 to February 2021 were selected as study objects.They were divided into the treatment group and the control group according to random number table method,with 45 cases in each group.The control group was treated with conventional drugs,while the treatment group was treated with pure moxibustion based on the treatment of the control group.The treatment efficacy,pain,Western Ontario and McMaster University Osteoarthritis Index(WOMAC)scores,time of five-times-sit-to-stand test(FTSST)and change in infrared thermal imaging before and after treatment were observed in both groups.Results The total effective rate of treatment in the treatment group was higher than that in the control group(93.33%vs.73.33%),with statistically significant difference(P<0.05).There were no statistically significant differences in the visual analogue scale(VAS)score,total WOMAC score,and FTSST time before treatment between the two groups(P>0.05).The VAS score,total WOMAC score,and FTSST time after treatment were lower than those before treatment in the two groups,and the anterolateral knee joint temperature after treatment was higher than that before treatment,with statistically significant differences(P<0.05).The VAS score,total WOMAC score and FTSST time in the treatment group were lower than those in the control group,but the anterolateral knee joint temperature was higher than that in the control group,with statistically significant differences(P<0.05).Conclusion The pain and knee function of KOA patients can significantly get relieved and improved,and FTSST time can get shortened after pure moxibustion treatment.Besides,the application of infrared thermal imaging can observe the change of knee joint temperature of patients,which can further reflect the clinical treatment effect and provide an objective basis for the efficacy of pure moxibustion in the treatment of KOA.
