中医康复护理模式在慢性心力衰竭患者中的应用

Application of traditional Chinese medicine rehabilitation nursing model in patients with chronic heart failure

目的探讨中医康复护理模式在慢性心力衰竭患者中的应用效果。方法选择2020年4月至2020年8月就诊于南昌大学第一附属医院心内科的80例慢性心力衰竭患者,采用随机数字表法进行分组,试验组、对照组各40例,对照组实施常规护理方法,试验组在对照组的基础上添加中医康复护理模式。比较两组患者干预3个月后的心功能改善情况(心功能分级、6 min步行试验距离)、实验室检查指标情况[左心室射血分数(LVEF)、N末端脑钠肽前体(NT-proBNP)]、生活质量及心力衰竭自我管理情况。结果干预3个月后,试验组心功能分级、NT-proBNP均低于对照组,差异有统计学意义(P<0.05);试验组6 min步行试验距离、LVEF及心力衰竭自我管理评分均高于对照组,差异有统计学意义(P<0.05);试验组生活质量评分低于对照组,差异有统计学意义(P<0.05)。结论应用中医康复护理模式能够提高慢性心力衰竭患者活动耐力并改善其心功能情况,能有效提升患者生活质量与自我管理能力,在促进患者心功能恢复和提高生存质量中起到积极影响。

Objective To explore the effect of traditional Chinese medicine(TCM)rehabilitation nursing model applied in patients with chronic heart failure.Methods A total of 80 patients with chronic heart failure who attended the Department of Cardiology of the First Affiliated Hospital of Nanchang University from April 2020 to August 2020 were selected as study objects and randomly divided them into the control group and the experimental group according to the random number method,with 40 cases in each group.The control group implemented conventional nursing model,while the experimental group used the TCM rehabilitation nursing model to intervene in the patients on the basis of the control group.The cardiac function classification of New York Heart Association(NYHA),6-min walk test distance,left ventricular ejection fraction(LVEF),N-terminal brain natriuretic peptide precursor(NT-proBNP),quality of life and self-management condition of heart failure were compared between the two groups after 3 months of intervention.Results After 3 months of intervention,the cardiac function classification and NT-proBNP in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).The 6-min walk test distance,LVEF and heart failure self-management scores in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).The quality of life score in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of TCM rehabilitation nursing model can improve cardiac function,activity endurance,quality of life and self-management ability of patients with chronic heart failure,thus contributing to the recovery of cardiac function and quality of life of patients.
