首发精神分裂症患者家属的病耻感对患者复发率及诊疗依从性的影响

Impacts of family members of stigma of on relapse rate and compliance of patients with first-episode schizophrenia

目的探讨首发精神分裂症患者家属的病耻感对患者复发率及依从性的影响分析。方法前瞻性分析江门市新会区第三人民医院精神科于2018年2月至2020年2月收治的首发精神分裂症患者家属共70例,按照随机数字表法进行分组,其中观察组35例,对照组35例,分析两组患者的病耻感差异,比较两组患者的反复发作率和过程中的依从性。结果观察组患者家属的歧视、疏远、社交躲避、病耻感抵触改善状况明显优于对照组,差异有统计学意义(P<0.05);对照组患者的反复发作率为5.71%,而观察组患者为14.29%,差异有统计学意义(χ^(2)=4.089、P=0.040);且对照组患者的反复发作率高于观察组,差异有统计学意义(P<0.05)。结论在首发精神分裂症患者的治疗中,应该对患者家属进行积极的宣传教育,端正对疾病的认识,消除患者及家属的消极心态,提高患者和家属的治疗依从性,促进患者早日康复。

Objective To analyze the impacts of family members of stigma on relapse rate and compliance of patients with first-episode schizophrenia(FES).Methods A total of 70 family members of patients with FES admitted to The Third People's Hospital of Xinhui District in Jiangmen City from February 2018 to February 2020 were prospectively analyzed.Patients were divided according to the random number table method,with 35 patients in the observation group and 35 patients in the control group.The differences in stigma between the two groups were analyzed,and the relapse rate and compliance of patients in the treatment process were compared between the two groups.Results The improvement of family members'discrimination,alienation,social avoidance and stigma resistance in the observation group was significantly better than that in the control group(P<0.05).The relapse rate of patients in the control group was 5.71%,while that of patients in the observation group was 14.29%,thus it could be determined that there was statistically significant difference between the two groups(χ^(2)=4.089,P=0.040).In addition,the relapse rate of patients in the control group was far higher than that of patients in the observation group,with statistically significant difference(P<0.05).Conclusion In the treatment of patients with FES,positive publicity and education should be carried out to the family members of patients to correct their understanding of the disease in order to eliminate the negative mentality of patients and their family members,improve the treatment compliance of patients and their family m embers,and promote the early recovery of patients.
