女性外阴获得性淋巴管瘤9例临床及组织病理分析

Clinicopathologic Analysis of 9 Cases of Female Acquired Lymphangiectasia of the Vulva

目的探讨女性外阴获得性淋巴管瘤临床、病理特征及鉴别诊断。方法对2016年1月-2020年12月确诊的本院临床诊治过程中9例外阴获得性淋巴管瘤患者临床、病理特征进行回顾性分析。结果9例外阴淋巴管瘤患者皮损主要分布于双侧大阴唇,表现为多发粟粒至绿豆大小透明丘疹、丘疱疹、水疱,偶有瘙痒,部分搔抓后有透明液体流出。病理表现为真皮浅层扩张的淋巴管腔,免疫组织化学显示扩张的淋巴管内皮细胞D2-40呈阳性表达。结论外阴部位的淋巴管瘤临床罕见,临床医生缺乏对本病的认识导致误诊漏诊。行皮肤组织病理检查可以及早明确诊断,避免误诊漏诊。

Objective To investigate the clinical,pathological features and differential diagnosis of female acquired lymphangiectasia of the vulva.Methods The clinical and pathological features of nine female patients with acquired lymphangiectasia of the vulva who were diagnosed in our hospital from January 2016 to December 2020 were analyzed retrospectively.Results The lesions of nine acquired lymphangiectasia of the vulva patients were mainly distributed on the bilateral labia majora and showed multiple miliary to mung bean sized clear papules,papulovesicle,and blisters,with occasional itching and clear fluid discharge after scratching.Histopathological examination revealed dilated lymphatic lumina in the superficial dermis,and immunohistochemistry showed that the dilated lymphatic endothelial cells were positively expressed for D2-40.Conclusion The clinical diagnosis of lymphangiomas of the vulvar is relatively rare.The lack of understanding of this disease by clinical dermatologists leads to misdiagnosis.Skin histopathological examination contributes to a clear diagnosis in order to avoid misdiagnosis.
