美沙拉秦联合双歧杆菌活菌对溃疡性结肠炎患者疗效、炎性因子、肠道菌群及粪便相关因子的影响 被引量:11

摘要 目的探讨美沙拉秦联合双歧杆菌活菌对溃疡性结肠炎(UC)患者疗效、炎性因子、肠道菌群及粪便相关因子的影响。方法选取2019年4月—2020年3月收治的UC 120例。根据治疗方法的不同分为观察组61例和对照组59例,观察组给予美沙拉秦联合双歧杆菌活菌治疗,对照组给予美沙拉秦治疗,均治疗3个月。比较两组临床疗效,治疗前后血清C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平、肠道有益菌群数量及粪便基质金属蛋白酶-9(MMP-9)、钙卫蛋白(Cal)、髓过氧化物酶(MPO)水平,同时观察不良反应发生情况。结果观察组总有效率高于对照组(P<0.05)。治疗后,两组血清CRP、IL-6、TNF-α水平低于治疗前,且观察组低于对照组(P<0.01)。治疗后,两组肠道乳酸菌、双歧杆菌数量高于治疗前,且观察组高于对照组(P<0.01)。治疗后,两组粪便中MMP-9、Cal、MPO水平均低于治疗前,且观察组低于对照组(P<0.01)。两组治疗期间不良反应总发生率比较差异无统计学意义(P>0.05)。结论美沙拉秦联合双歧杆菌活菌可提高UC患者疗效,减轻炎症反应程度,改善肠道微生态环境,且治疗安全性好。 Objective To investigate the effects of Mesalazine combined with live bifidobacteria on efficacy,inflammatory factors,intestinal flora and feces-related factors in treatment of patients with ulcerative colitis(UC).Methods A total of 120 patients with UC admitted between April 2019 and March 2020 was selected and divided into observation group(n=61)and control group(n=59)according to different therapeutic methods.Observation group was treated with Mesalazine combined with live bifidobacteria,while control group was treated with Mesalazine.Both groups were treated for 3 months.Clinical efficacy,levels of serum C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),the number of beneficial intestinal flora,levels of matrix metalloproteinases-9(MMP-9)and calprotectin(Cal),myeloperoxidase(MPO)in feces before and after treatment were compared,and incidence rates of adverse reactions were observed in two groups.Results The total effective rate in observation group was significantly higher than that in control group(P<0.05).After treatment,serum levels of CRP,IL-6 and TNF-αwere significantly higher than those before treatment in two groups,and the levels in observation group were significantly lower than those in control group(P<0.01).After treatment,numbers of intestinal lactic acid bacteria and bifidobacteria were significantly higher than those before treatment in two groups,and the numbers in observation group were significantly higher than those in control group(P<0.01).After treatment,levels of MMP-9,Cal and MPO in feces were significantly lower than those before treatment in two groups,and the levels in observation group were significantly lower than those in control group(P<0.01).There was no significant difference in the total incidence rate of adverse reactions between two groups(P>0.05).Conclusion Mesalazine combined with live bifidobacteria may improve the curative effect,reduce the degree of inflammatory reaction and improve the intestinal microecological environment with good safety.
