摘要
目的:探讨胰十二指肠切除术后早期肠内营养联合应用大承气汤对患者康复的影响。方法:选择2017年1月—2019年12月入住我院肝胆胰外科并行胰十二指肠切除术的患者38例,随机分为对照组18例与试验组20例,所有患者术后早期均给予肠内营养,试验组加用大承气汤200 mL/d。观察并比较两组患者术后胃肠功能及肝功能等恢复情况。结果:与对照组比,试验组患者术后肛门排气时间明显缩短[(50.24±3.39)h vs(67.35±5.45)h],差异有统计学意义(P<0.05),术后第4天的前白蛋白、术后第7天的白蛋白与总胆红素指标恢复更明显,差异有统计学意义(P<0.05)。结论:胰十二指肠切除术后早期肠内营养联合应用大承气汤能促进患者胃肠道功能恢复,更有效促进患者营养状况恢复,改善患者预后。
Objective To explore the effect of early enteral nutrition combined with Dachengqi Decoction on patients'rehabilitation after pancreaticoduodenectomy.Methods Thirty-eight patients undergoing pancreaticoduodenectomy were randomly divided into two groups,which 18 cases in the control group and 20 cases in the test group.All patients were given enteral nutrition early after surgery.The test group was given Dachengqi Decoction 200 mL/d.The recovery of gastrointestinal function and liver function of the two groups were observed and analyzed.Results Compared with the control group,the postoperative anal exhaust time was significantly shorter in the experimental group(P<0.05),and the indexes of prealbumin on the 4th day after the operation and the albumin and total bilirubin on the 7th day after the operation were recovered more obviously.The difference was statistically significant(P<0.05).Conclusion Early enteral nutrition after pancreaticoduodenectomy combined with the application of Dachengqi Decoction can promote the recovery of the patient’s gastrointestinal function,more effectively promote the recovery of the patient's nutritional status and improve the patient’s prognosis.
