枢经推拿治疗腰椎间盘突出症的临床疗效及对患者病变区域红外热成像的影响

Clinical efficacy of pivot meridians massage for treatment of lumbar disc herniation and its effect on infrared thermal imaging of patients′lesion area

目的观察枢经推拿治疗腰椎间盘突出症的临床疗效及对患者病变区域红外热成像的影响。方法60例腰椎间盘突出症患者均采用枢经推拿治疗,共治疗2个疗程。观察患者治疗前后改良日本骨科学会(下腰痛)评分(mJOA)、疼痛视觉模拟量表(VAS)评分,以及足太阳膀胱经(腰骶部)、足少阳胆经(下肢神经支配区域)及足太阴肾经(足底)红外热图的温度变化情况。结果60例患者中痊愈17例、显效27例、有效12例、无效4例,总有效率为93.33%(56/60)。与治疗前比较,患者治疗后的mJOA升高,VAS评分降低,并且患者足太阳膀胱经(腰骶部)皮肤温度降低,患侧足少阳胆经(下肢神经支配区域)及足太阴肾经(足底)皮肤温度升高(均P<0.05)。结论枢经推拿能够有效地改善腰椎间盘突出症患者的临床症状,疗效确切。红外热像成像技术或可成为诊断腰椎间盘突出症及判断其治疗效果的有效方法。

Objective To observe the clinical efficacy of pivot meridians massage for the treatment of lumbar disc herniation and its effect on infrared thermal imaging of patients′lesion area.Methods Sixty patients with lumbar disc herniation were treated with pivot meridians massage for two courses of treatment.The Modified Japanese Orthopedics Association Score(mJOA)for lower back pain,Visual Analogue Scale(VAS)pain scores,and temperature variances on infrared thermogram for bladder meridian of foot-Taiyang(lumbosacral),gallbladder meridian of foot-Shaoyang(lower limb innervation area)and kidney meridian of foot-Taiyin(planta pedis)of the patients were observed before and after treatment.Results Among the 60 cases,17 cases were cured,27 cases were markedly effective,12 cases were effective,four cases were ineffective,and the total effective rate was 93.33%(54/60).Compared with those before treatment,mJOA was increased,VAS score was decreased,skin temperature along bladder meridian of foot-Taiyang(lumbosacral)was lower,and skin temperature along gallbladder meridian of foot-Shaoyang(lower limb innervation area)or kidney meridian of foot-Taiyin(planta pedis)on affected side was higher after treatment(all P<0.05).Conclusion pivot meridians massage can effectively improve clinical symptoms in patients with lumbar disc herniation with a definite efficacy.Infrared thermal imaging technique may be an effective method for diagnosing lumbar disc herniation and judging its therapeutic effect.
