摘要
目的探讨联合应用瞬态诱发性耳声发射(transient evoked otoacoustic emission,TEOAE)和自动听性脑干反应(automated auditory brainstem response,AABR)在新生儿听力筛查中的临床意义。方法对2020年1月至2020年12月在徐州市妇幼保健院出生的足月健康新生儿7169例出院之前进行TEOAE初筛,初筛未通过者在出生后42d应用AABR进行复筛。复筛未通过者于3月龄转诊至徐医附院听力障碍诊断中心进行诊断性检查。结果在初始的听力筛查中,共有361名新生儿未通过TEOAE测试,初筛未通过率为5.03%,其中276名婴儿于42天再用AABR进行复筛,有33人复筛未通过,经过听力诊断,最终有9人被确诊为不同程度的听力损失。结论AABR具有灵敏、客观、无创等优点,是一种比较好的新生儿听力筛查方法,TEOAE与AABR联合应用可以优势互补,降低误诊率,对新生儿听力损失做到早发现、早干预,有利于提高新生儿语言和认知能力。
Objective The aim of this study is to assess the clinical applications of the combined use of TEOAE and AABR in universal newborn hearing screening.Methods The clinical data of babies from January 2020 to December 2020 were reviewed and analyzed.These infants who failed the initial hearing screening in the hospital need received AABR tests 42 days after birth.Those who failed the re-screening were referred to the hearing Impairment Diagnostic Center of Affiliated Hospital of Xuzhou Medical University for diagnostic examination at the age of 3 months.Results In the initial hearing screening,a total of 361 newborns failed TEOAE test,and the initial screening failure rate was 5.03%.Among them,276 infants were re-screened with AABR,and 33 infants failed re-screening.After hearing diagnosis,9 infants were finally diagnosed with varying degrees of hearing loss.Conclusion The combined use of TEOAE and AABR in universal newborn hearing screening are complementary each other in screening,and will enhance the accuracy of hearing screening.
