蛇毒血凝酶注射液联合垂体后叶素治疗剖宫产产后出血的效果

Effect of Hemocoagulase Injection Combined with Pituitrin in the Treatment of Postpartum Hemorrhage After Cesarean Section

目的探讨垂体后叶素联合蛇毒血凝酶注射液治疗剖宫产产后出血(PPH)的效果。方法选取2019年4月至2021年4月驻马店市中心医院收治的88例剖宫产PPH患者作为研究对象,按随机抽签法分为对照组和观察组,各44例。在常规治疗的基础上,对照组接受垂体后叶素治疗,观察组接受蛇毒血凝酶注射液联合垂体后叶素治疗。比较两组用药2、24 h出血量;于用药前、用药后24 h,比较两组凝血功能[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)]以及子宫平滑肌收缩功能(频率、幅度、活动力);比较两组不良反应发生情况。结果观察组用药2、24 h的出血量较对照组少(P<0.05);用药24 h,两组PT、APTT、TT短于用药前,FIB小于用药前,且观察组PT、APTT、TT短于对照组,FIB小于对照组(P<0.05);用药后24 h,两组子宫平滑肌活动力、收缩幅度以及收缩频率均高于用药前,且观察组高于对照组(P<0.05);两组均未见明显不良反应。结论蛇毒血凝酶注射液联合垂体后叶素治疗剖宫产PPH的止血效果较好,可改善患者的凝血功能和子宫平滑肌收缩功能。

Objective To investigate the effect of Hemocoagulase injection combined with pituitrin in patients with postpartum hemorrhage(PPH)after cesarean section.Methods A total of 88 patients with PPH at cesarean section admitted to Zhumadian Central Hospital from April 2019 to April 2021 were selected as the research subjects.According to the random drawing lots method,they were divided into control group and observation group,with 44 cases in each group.On the basis of conventional treatment,the control group received pituitrin treatment,and the observation group received Hemocoagulase injection combined with pituitrin.The amount of bleeding at 2 hours and 24 hours after medication were compared between the two groups.The coagulation function[prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB)]and the contractile function of uterine smooth muscle(frequency,amplitude and activity)were compared between the two groups before and 24 hours after medication.The occurrence of adverse reactions was compared between the two groups.Results The amount of bleeding in the observation group at 2 hours and 24 hours after medication were less than that in the control group(P<0.05).At 24 hours after treatment,PT,APTT and TT were shorter than before the treatment,and FIB was less than before the treatment,and PT,APTT and TT of the observation group were shorter than the control group,and FIB was lower than the control group(P<0.05).At 24 hours after treatment,the activity force,contraction amplitude and contraction frequency of uterine smooth muscle in both groups were higher than before the treatment,and the observation group was higher than the control group(P<0.05).There were no obvious adverse reactions in both groups.Conclusion The hemostatic effect of Hemocoagulase injection combined with pituitrin has a better hemostatic effect in the treatment of PPH in cesarean section,and it can improve the blood coagulation function and uterine smooth muscle contraction function of patients.
