Changes and Clinical Significance of CD4^(+),CD8^(+),Erythrocyte Sedimentation Rate Levels in Patients with Systemic Lupus Erythematosus with Different Severity

目的探究不同病情程度系统性红斑狼疮(SLE)患者CD4^(+)、CD8^(+)、红细胞沉降率(ESR)水平变化及临床意义。方法选取2018年10月至2019年10月郑州大学第二附属医院收治的85例SLE患者作为观察组,再选取同期92例健康体检者为对照组,比较两组CD4^(+)、CD8^(+)、ESR水平,记录不同病情程度和预后的CD4^(+)、CD8^(+)、ESR水平,分析CD4^(+)、CD8^(+)、ESR水平与病情程度的相关性。结果观察组ESR、CD8^(+)水平高于对照组,CD4^(+)水平低于对照组(P<0.05);随病情程度加重,ESR、CD8^(+)水平逐渐升高,CD4^(+)水平逐渐降低(P<0.05);经Spearman相关分析,ESR、CD8^(+)水平与病情程度呈正相关,CD4^(+)水平与病情程度呈负相关(P<0.05);预后不良患者ESR、CD8^(+)水平高于临床缓解患者,CD4^(+)水平低于临床缓解患者(P<0.05)。结论SLE患者CD4^(+)、CD8^(+)、ESR水平会发生明显变化,且与病情程度及预后具有一定相关性,通过检测CD4^(+)、CD8^(+)、ESR水平可评估病情及预后情况,为临床完善治疗方案提供相关依据。

Objective To explore the changes and clinical significance of CD4^(+),CD8^(+),erythrocyte sedimentation rate(ESR)levels in patients with systemic lupus erythematosus(SLE)with different disease levels.Methods Eighty-five patients with SLE in the Second Affiliated Hospital of Zhengzhou University from October 2018 to October 2019 were selected as the observation group,and 92 healthy physical examination patients during the same period were selected as the control group.The levels of CD4^(+),CD8^(+) and ESR in the two groups were compared,and the levels of CD4^(+),CD8^(+),ESR with different severity and prognosis in the two groups were recorded,and the correlation of CD4^(+),CD8^(+),ESR levels and the severity of the disease were analyzed.Results The ESR and CD8^(+) levels of the observation group were higher than those of the control group,and the CD4^(+) level was lower than that of the control group(P<0.05).With the severity of the disease,the ESR and CD8^(+) levels gradually increased,and the CD4^(+) level gradually decreased(P<0.05).By Spearman correlation analysis shows that ESR and CD8^(+) levels were positively correlated with disease severity,and CD4^(+) levels were negatively correlated with disease severity(P<0.05).ESR and CD8^(+) levels in patients with poor prognosis were higher than those in clinical remission,and CD4^(+) levels were lower than those in clinical remission(P<0.05).Conclusion The CD4^(+),CD8^(+) and ESR levels of SLE patients will change significantly,and they have a certain correlation with the severity of the disease and the prognosis.The detection of CD4^(+),CD8^(+) and ESR levels can assess the disease and prognosis,and provide relevant evidence for clinical improvement of the treatment plan.
