早期综合护理干预在妊娠早期甲状腺功能减退症患者中的应用效果

Application Effect of Early Comprehensive Nursing Intervention on Patients with Hypothyroidism in Early Pregnancy

目的探讨早期综合护理干预在妊娠早期甲状腺功能减退症患者中的应用效果。方法选取2018年1月至2019年12月在平顶山市妇幼保健院接受孕检的230例妊娠早期合并甲状腺功能减退症患者作为研究对象,根据妊娠期间接受干预的情况分为干预组(127例)和对照组(103例)。两组患者均接受左甲状腺素钠片治疗,对照组接受常规孕检,干预组在此基础上,接受早期综合护理干预方案。比较两组患者依从性、妊娠结局和新生儿Apgar评分。结果干预组患者妊娠期间药物服用、孕检复查以及日常生活管理3个方面的依从性均高于对照组(P<0.05);干预组患者流产率和早产率低于对照组,并且干预组顺产率高于对照组(P<0.05);干预组新生儿出生1 min和5 min时的Apgar评分均高于对照组(P<0.05)。结论早期综合护理干预有利于改善妊娠早期甲状腺功能减退症患者的治疗依从性,减少流产、早产等并发症的发生,提高顺产率,可有效保障母婴健康。

Objective To explore the effect of early comprehensive nursing intervention on patients with hypothyroidism in early pregnancy.Methods A total of 230 cases of early pregnancy patients with hypothyroidism who underwent pregnancy examinations in MCH Hospital of Pingdingshan City from January 2018 to December 2019 were selected as the research subjects.According to the situation of receiving intervention during pregnancy,they were divided into intervention group(127 cases)and control group(103 cases).Both groups were treated with levothyroxine sodium tablets,the control group received routine pregnancy examination,and the intervention group received early comprehensive nursing intervention program on this basis.The compliance,pregnancy outcome and Apgar score were compared between the two groups.Results The compliance of intervention group in the three aspects of drug use,pregnancy examination and daily life management during pregnancy were higher than that of the control group(P<0.05).The abortion rate and premature birth rate in the intervention group were lower than that of the control group,and the natural birth rate in the intervention group was higher than that of the control group(P<0.05).The Apgar scores at 1 min and 5 min after birth in the intervention group were higher than that of the control group(P<0.05).Conclusion Early comprehensive nursing intervention could help to improve the treatment compliance of patients with hypothyroidism in early pregnancy,reduce the complication occurrence of abortion and premature birth,improve the birth rate,and effectively protect the health of mother and child.
