海战批量伤员快速建立静脉通路策略 被引量:2

First-line Rescue Strategy for Rapid Establishment of Intravenous Access for Mass Casualties in Naval Warfare

摘要 目的通过平战时海上特殊环境下批量伤员救治中快速建立静脉通路技术的应用实践经验,结合卫勤分队海上战创伤技术救护流程,研究探索海战批量伤员快速建立静脉通路一线救护流程策略。方法采用回顾性分析将某部60例模拟伤员分为对照组和实验组,各30例,分别模拟海战批量伤员救治需求建立静脉输液通路。对照组按照陆基医院常规救治流程模式进行操作,实验组则由医护人员按照海上特殊环境下应急救护流程对重症伤员进行静脉通路建立,比较两组重症伤员救治过程中操作时间、成功率、效果满意率和专家认可度。结果实验组伤员操作用时较对照组短,成功率和专家认可度均比对照组高,差异具有统计学意义(P<0.01)。结论海战批量伤员快速建立静脉通路一线救护策略可以克服光线不足、低血容量性休克伤员或海水浸泡伤员肢体静脉不易充盈等问题,结合战创伤救护流程能够为战时批量伤员救护快速建立静脉通路、打通生命救护通道提供实践经验和技术保障。 Objective Based on the application and practical experience of rapid establishment of intravenous access for mass casualties treatment in peacetime and wartime Marine special environment, and combined with the technical rescue process of Marine combat trauma of medical service detachment, this article explored the first-line rescue strategy for rapid establishment of intravenous access technology for mass casualties in naval battles. Methods Through retrospective analysis, 60 cases of simulated wounded in a hospital were divided into the control group and the experimental group, with 30 cases in each group. The control group was operated in accordance with the routine treatment procedures of land-based hospitals, while the experimental group was operated by medical staff in accordance with the emergency rescue procedures in special Marine environment. The operation time, success rate, satisfaction rate and expert recognition of the treatment of severe patients in the two groups were compared. Results The operation time in the experimental group was shorter than that in the control group, and the success rate and expert recognition were higher than those of the control group. The differences were statistically significant(P<0.01). Conclusion The first-line rescue strategy for rapid establishment of intravenous access for mass casualties can overcome such weaknesses as insufficient light and difficulties in filling limb veins of the wounded with hypovolemic shock or seawater immersion. Combined with the rescue and treatment procedure of war trauma, it can provide practical experience and technical support for the rapid establishment of intravenous access and the opening of life rescue channel for the mass casualties in war.
