摘要
目的探讨外周血中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)对变应性鼻炎患者病情严重程度的预测价值。方法选取本院2016年1月至2019年12月收治的137例变应性鼻炎患者作为病例组,根据世界卫生组织对过敏性鼻炎的分度标准将患者分为轻度组(52例)、中度组(56例)和重度组(29例);选取同期65名健康体检者作为对照组。分析并比较各组的NLR、PLR;采用受试者工作曲线(ROC)分析NLR、PLR对变应性鼻炎患者病情严重程度的预测价值。结果病例组的NLR、PLR明显高于对照组(P<0.05)。不同病情变应性鼻炎患者的NLR、PLR比较,差异具有统计学意义(P<0.05),且随着病情严重程度的增加呈升高趋势。外周血NLR、PLR的最佳截断值为1.98、127.92,诊断中度和重度变应性鼻炎患者的AUC为0.731(95%CI:0.640~0.821)、0.762(95%CI:0.681~0.843),灵敏度为71.16%、70.59%,特异度为73.08%、82.69%。结论变应性鼻炎患者外周血NLR、PLR上升,与病情严重程度密切相关,可作为预测病情严重程度的标志物。
Objective To explore the predictive value of peripheral blood neutrophil to lymphocyte ratio(NLR)and platelet to lymphocyte ratio(PLR)in the severity of illness of allergic rhinitis patients.Methods A total of 137 patients with allergic rhinitis admitted in our hospital from January 2016 to December 2019 were selected as case group,according to the World health organization's grading standard for allergic rhinitis,the patients were divided into mild group(52 cases),moderate group(56 cases)and severe group(29 cases);a total of 65 healthy subjects in the same period were selected as control group.The NLR and PLR of each group were analyzed and compared;the receiver operating curve(ROC)was used to analyze the predictive value of NLR and PLR for the severity of illness of allergic rhinitis patients.Results The NLR and PLR in the case group were significantly higher than those in the control group(P<0.05).The NLR and PLR of patients with allergic rhinitis in different conditions were statistically significant(P<0.05),and showed an upward trend with the increase in severity of illness of the disease.The best cut-off values of peripheral blood NLR and PLR were 1.98 and 127.92,and the AUC for the diagnosis of moderate and severe allergic rhinitis were 0.731(95%CI:0.640-0.821)and 0.762(95%CI:0.681-0.843),the sensitivity were 71.16%,70.59%,and the specificity were 73.08%,82.69%.Conclusion The peripheral blood NLR and PLR in patients with allergic rhinitis increase and are closely related to the severity of illness of the disease,and they can be used as markers to predict the severity of the disease.
