精细化护理干预在老年食管癌全肠内营养支持患者中的应用效果

Application effect of refined nursing intervention in total enteral nutrition support for elderly patients with esophageal cancer

目的探究精细化护理干预对老年食管癌全肠内营养(TEN)支持患者营养状态和预后的影响。方法将86例在本院接受治疗的老年食管癌患者采用随机数字表法分为对照组和研究组,各43例。对照组术后接受常规对症处理和TEN支持,研究组在对照组对症处理及TEN支持过程中开展精细化护理干预。比较两组干预效果。结果干预后,研究组ALB、Hb水平及BMI高于对照组,PG-SGA评分低于对照组,预期目标喂养量达标时间短于对照组(P<0.05)。研究组干预期间的恶心呕吐、腹泻、吸入性肺炎发生率低于对照组(P<0.05)。研究组的生存时间明显长于对照组(P<0.05)。结论老年食管癌患者TEN支持过程中开展精细化护理干预有助于改善患者营养状况,延长生存期。

Objective To explore the effects of refined nursing intervention on nutritional status and prognosis in total enteral nutrition(TEN)support for elderly patients with esophageal cancer.Methods A total of 86 elderly patients with esophageal cancer treated in our hospital were divided into control group and study group by random number table method,with 43 cases in each group.The control group was given conventional symptomatic treatment and TEN support after operation,the study group carried out refined nursing intervention in the process of symptomatic treatment and TEN support of control group.The intervention effects of the two groups were compared.Results After intervention,the ALB,Hb levels and BMI in the study group were higher than those in the control group,the PG-SGA score was lower than that in the control group,the time of reaching the standard of the expected target feeding amount was shorter than that in the control group(P<0.05).The incidence of nausea,vomiting,diarrhea and aspiration pneumonia during intervention in the study group were lower than those in the control group(P<0.05).The survival time of the study group was significantly longer than that of the control group(P<0.05).Conclusion Refined nursing intervention in the process of TEN support of elderly patients with esophageal cancer is helpful to improve their nutritional status and prolong their survival time.
