血液净化患者专科护理质量评价指标体系的构建研究

Study on Construction of Evaluation Index System of Specialized Nursing Quality for Patients with Blood Purification

目的构建血液净化患者专科护理质量评价指标体系,为临床血液净化专科护士血液透析治疗提供全面、系统、可量化的参考依据。方法借助Donabedian的“结构-过程-结果”三维质量评价工具,采取文献分析、问卷调查研究、德尔菲法(Delphi)等方法,以电子邮件、半结构式访谈、快递或电话等方式发送问卷咨询表,经过2轮专家咨询法拟定我国血液净化患者专科护理质量评价指标体系相关内容。结果2轮专家函询后Cr值均在0.85~0.98范围,重要性赋值均数≥4.5,标准偏差在0.10~0.15范围,CV分别为0.201、0.179(P<0.01)。2轮研究有效回收率分别为100%、88.89%。最终确立涵盖3项一级指标,9项二级指标,50项三级指标体系。结论此次咨询专家的积极性、权威性、意见集中程度、意见协调程度均处于较高水平,拟定的血液净化患者专科护理质量评价指标体系对国内医疗机构血液净化的实施、培训、质控均具有良好参考借鉴作用。

Objective To construct the evaluation index system of nursing quality of blood purification patients,and provide comprehensive,systematic and quantifiable reference basis for hemodialysis treatment of clinical blood purification specialist nurses.Methods Through the Donabedian"structure-process-results"three-dimensional quality evaluation tools,using literature analysis,questionnaire research,Delphi method(Delphi)and other methods,e-mail,semi-structured visits,express delivery,or telephone and other forms of questionnaire consultation through two rounds of expert consultation method to draw up the evaluation index system of specialized nursing quality of patients with blood purification in China.Results The Cr values were in the range of 0.85~0.98,it shows that expert authority and reliability are high.Average of importance assignment≥4.5,the standard deviation was in the 0.10~0.15 range,It indicated a high concentration of expert opinions.CV 0.201,0.179,P<0.01,Less volatile,It shows that the coordination of experts is high.In addition,the consultant in this study pays great attention to the investigation and display.The effective recovery rates of the corresponding 2 rounds were 100%and 88.89%respectively.Conclusion This consultation expert's enthusiasm,authority,the opinion concentration degree,the opinion coordination degree are all in the high level,the proposed blood purification patient specialized nursing quality appraisal index system has the good reference function to the domestic medical institution blood purification implementation,the training,the quality control.
