经阴道三维超声子宫冠状面重建在子宫发育异常分型诊断中的应用价值

Application value of transvaginal three-dimensional ultrasound in the diagnosis of uterine dysplasia

目的分析经阴道三维超声子宫冠状面重建在子宫发育异常分型诊断中的应用价值。方法选取本院2016年2月至2020年3月收治的400例疑似子宫发育异常患者,按照入院治疗的先后顺序分为对照组与研究组,每组200例。对照组接受二维超声检查,研究组接受经阴道三维超声检查。以经腹腔镜检查诊断作为金标准,分析二维超声和三维超声检查的分型诊断价值。结果经腹腔镜检查结果显示,400例疑似患者中,45例患者正常,355例患者异常,其中60例患者为双子宫,30例患者为弓形子宫,17例患者为残角子宫,27例患者为单角子宫,184例患者为纵膈子宫(121例为不完全型,63例为完全型),37例患者为双角子宫。二维超声检查诊断异常患者367例,三维超声检查诊断异常患者356例。三维超声检查的准确率、敏感性、特异性均明显高于二维超声检查,比较差异有统计学意义(P<0.05)。结论经阴道三维超声子宫冠状面重建分型诊断子宫发育异常的准确率较高,能为临床诊断和治疗提供准确的参考依据,值得临床推广运用。

Objective To analyze the application value of transvaginal three-dimensional ultrasound in the diagnosis of uterine dysplasia.Methods 400 cases of suspected uterine dysplasia in our hospital from February 2016 to March 2020 were selected and they were divided into control group and study group according to the order of admission treatment,with 200 cases in each group.The control group received two-dimensional ultrasound examination,the study group received transvaginal three-dimensional ultrasound examination.The diagnostic value of two-dimensional ultrasound and three-dimensional ultrasound was analyzed.Results 400 suspected patients,45 patients were normal,355 patients were abnormal,including 60 patients with double uterus,30 patients with arcuate uterus,17 patients with residual horn uterus,27 patients with single horn uterus,184 patients with mediastinal uterus(121 cases with incomplete type,63 cases with complete type),37 cases with bicornate uterus.The accuracy,sensitiv-ity and specificity of three-dimensional ultrasound were significantly higher than those of two-dimensional ultrasound(P<0.05).Conclusion The accuracy rate of transvaginal three-dimensional ultrasound coronal reconstruction in the diagnosis of uterine dysplasia is high,which can provide accurate and rich reference basis for clinical diagnosis and treatment,and it is worth of clinical promotion.
