经乳晕入路完全腔镜下甲状腺切除术治疗良性甲状腺病变的效果及安全性观察

Observation on the efficacy and safety of complete laparoscopic thyroidectomy through areola approach in the treatment of benign thyroid diseases

目的观察经乳晕入路完全腔镜下甲状腺切除术治疗良性甲状腺病变的效果及安全性。方法回顾性分析2017年10月至2019年10月于本院进行手术治疗的80例良性甲状腺病变患者的临床资料,按照手术方式的不同分为对照组和观察组,各40例。对照组采用开放性甲状腺切除术进行治疗,观察组采用经乳晕入路完全腔镜下甲状腺切除术进行治疗。比较两组患者术后切口美观度、各项手术相关指标、术后复发率及甲状腺功能损伤情况。结果术后30、60 d,观察组NSS评分均显著高于对照组(P<0.05);观察组手术时间、术后引流时间及住院时间均明显短于对照组,且术中出血量及术后引流量明显少于对照组(P<0.05);观察组术后复发率和甲状腺功能损伤率均低于对照组(P<0.05)。结论对良性甲状腺病变患者采取经乳晕入路完全腔镜下甲状腺切除术进行治疗能减少术中出血量,缩短住院时间,降低术后复发率、甲状腺功能损伤率,手术切口较小,美观度高,且安全性较高,值得临床推广运用。

Objective To observe the efficacy and safety of complete laparoscopic thyroidectomy via areola approach in the treatment of benign thyroid diseases.Methods A retrospective analysis of the clinical data of 80 patients with benign thyroid disease who underwent surgical treatment in our hospital from October 2017 to October 2019,and they were divided into control group and observation group according to different surgical meth-ods,with 40 cases in each group.The control group was treated with open thyroidectomy,and the observation group was treated with complete laparo-scopic thyroidectomy through the areola approach.The postoperative incision aesthetics,various surgical indicators,postoperative recurrence rate and thyroid function damage were compared between the two groups.Results At 30 and 60 days after operation,the NSS scores of the observation group were significantly higher than those of the control group(P<0.05);the operation time,postoperative drainage time and hospital stay of the observation group were significantly shorter than those of the control group,and the amount of intraoperative blood loss and postoperative,drainage volume were significantly lower than those of the control group(P<0.05);the postoperative recurrence rate and the thyroid function damage rate of the observation group were significantly lower than those of the control group(P<0.05).Conclusion For patients with benign thyroid disease,complete laparoscopic thyroidectomy through the areola approach can reduce intraoperative blood loss,shorten hospital stay,reduce postoperative recurrence rate,thyroid function damage rate,small surgical incision,and high aesthetics,and the safety is high,it is worthy of clinical application.
