喷砂及手器刮治清除龈下菌斑效果的临床随机对照研究

A clinical randomized controlled study on the effect of sandblasting and hand scraping in removing subgingival plaque

目的分析喷砂及手器刮治清除龈下菌斑的效果。方法选取2019年7月至2020年6月本院收治的慢性牙周炎患者120例,随机分为喷砂刮治组和器械刮治组,各60例。器械刮治组采用超声龈下+手工刮治,喷砂刮治组在器械刮治组基础上结合龈下喷砂,比较两组患者菌斑指数变化情况、龈沟液中炎症因子(hs-CRP、IL-6、IL-10)水平。结果治疗前,两组患者菌斑指数、龈沟液中炎症因子水平比较差异无统计学意义;治疗后1周,喷砂刮治组患者菌斑指数、hs-CRP、IL-6均低于器械刮治组,IL-10高于器械刮治组(P<0.05)。结论与单纯超声龈下+手工刮治相比,在此基础上加入龈下喷砂对龈下菌斑的清除效果更显著,能有效抑制慢性牙周炎患者的牙周局部炎症反应。

Objective To analyze the effect of subgingival and hand scraping in removing subgingival plaque.Methods 120 patients with chron-ic periodontitis admitted to our hospital from July 2019 to June 2020 were randomly divided into subgingival scraping group and instrument scraping group,with 60 cases in each group.The instrument scraping group was treated ultrasonographic subgingival scaling+manual scaling.The ubgingival scraping group was combined with sandblasting on the basis of scraping group,and the changes of plaque index and the levels of inflammatory fac-tors(hs-CRP,IL-6,IL-10)in gingival crevicular fluid were compared between the two groups.Results Before treatment,there was no significant difference in plaque index and inflammatory factors in gingival crevicular fluid between two groups.1 week after treatment,plaque index,hs-CRP and IL-6 in subgingival scraping group were lower than those in instrument scraping group,and IL-10 was higher than that in instrument scraping group(P<0.05).Conclusion Compared with simple ultrasonic subgingival scaling and manual scaling,adding subgingival sandblasting on this ba-sis has a more significant effect on the removal of subgingival plaque,and can effectively inhibit the local periodontal inflammatory response in pa-tients with chronic periodontitis.
