左西孟旦联合多巴酚丁胺对老年急性心力衰竭患者心功能及炎症相关指标的影响

Effects of levosimendan combined with dobutamine on cardiac function and inflammatory indicators in elderly patients with acute heart failure

目的探讨左西孟旦联合多巴酚丁胺对老年急性心力衰竭患者心功能及炎症相关指标的影响。方法选取2019年1月至2020年1月在该院接受治疗的老年急性心力衰竭患者165例,分为联合组(83例)和单药组(82例)。单药组给予多巴酚丁胺治疗,联合组给予左西孟旦联合多巴酚丁治疗,比较两组临床疗效、血清氨基末端B型利钠肽前体(NT-proBNP)、C反应蛋白(CRP)、降钙素原(PCT)、中性粒细胞与淋巴细胞比值(NLR)等。结果治疗后,联合组总有效率高于单药组,差异有统计学意义(P<0.05);联合组血清NT-proBNP、CRP、PCT、NLR低于单药组,左室射血分数(LVEF)、短轴缩短率(FS)、6 min步行距离均高于单药组,左室舒张末内径(LVIDd)、呼吸频率均低于单药组,差异均有统计学意义(P<0.05)。结论左西孟旦联合多巴酚丁胺治疗老年急性心力衰竭患者疗效显著,可有效改善患者心功能及炎症状态。

Objective To study the effect of levosimendan combined with dobutamine on cardiac function and inflammatory indicators in elderly patients with acute heart failure.Methods A total of 165 elderly patients with acute heart failure who received treatment in the hospital from January 2019 to January 2020 were divided into the combined group(83 cases)and the monotherapy group(82 cases).The monotherapy group was treated with dobutamine,and the combined group was treated with levosimendan combined with dobutamine.The clinical efficacy,serum NT-proBNP,CRP,PCT and NLR were compared.Results After treatment,the total effective rate of the combined group was higher than that of the monotherapy group,and the difference was statistically significant(P<0.05).Serum NT-proBNP,CRP,PCT and NLR in the combined group were lower than those in the monotherapy group,and the LVEF,FS and 6 min walking distance were higher than those in the monotherapy group,while the LVIDd and respiratory rate were lower than those in the monotherapy group,and the differences were statistically significant(P<0.05).Conclusion Levosimendan combined with dobutamine in the treatment of elderly patients with acute heart failure has a significant effect and could effectively improve the cardiac function and inflammatory status of patients.
