基于“清浊相干”理论探讨脾胃湿热证与肠道菌群自稳态调控关系

Discussion on Relationship Between Spleen-stomach Dampness-heat Syndrome and Intestinal Flora Homeostasis Regulation Based on Theory of“Disorder Between Qing and Zhuo”

肠道菌群是消化系统的重要组成部分,因其自稳态调控功能在各器官系统的功能调节中起着重要作用,故肠道菌群自稳态失衡与临床多种疾病的发生密切相关。脾胃湿热证是中医脾胃证候的重要证型之一。所谓“清浊相干”,是水谷精微与糟粕不归正化、不行其道,气机升降逆乱的病机概括,是脾胃湿热证与肠道菌群自稳态失衡的关键环节。据此,本文提出对脾胃湿热相关的肠道菌群自稳态失衡,应立足脾胃,采用清热祛湿、燮理气机、平调寒热为主要治法,以恢复清浊协调状态。从“清浊相干”理论探讨脾胃湿热证与肠道菌群自稳态调控的关系,可进一步深化脾胃湿热证的理论内涵,为今后脾胃湿热证的现代研究提供新的临床思路和理论依据。

Intestinal flora is an important part of digestive system.Because of its homeostasis regulation function,it plays an important role in the regulation of functions of various organ system of human body.Therefore,the imbalance of intestinal flora homeostasis is closely related to the occurrence of various clinical diseases.Spleen-stomach dampness-heat syndrome is one of the important syndrome types of spleen and stomach in TCM.The so-called“disorder between qing and zhuo”is a summary of the pathogenesis of cereal essence and waste metabolic disturbance and the imbalance of qi ascending and descending.It is also a key link between the spleen-stomach dampness-heat syndrome and instability of intestinal flora.Accordingly,for the homeostasis imbalance of intestinal flora related to spleen-stomach dampness-heat syndrome,the spleen and stomach organs should be based on.The main clinical methods of differentiation and treatment are to clear heat and remove dampness,cooperate with qi movement,and adjust the cold and heat mildly,in order to restore the state of qing and zhuo.Discussing the relationship between spleen-stomach dampness-heat syndrome and intestinal flora homeostasis regulation from the theory of“disorder between qing and zhuo”can further deepen the theoretical connotation of spleen-stomach dampness-heat syndrome,which can provide new clinical ideas and theoretical basis for the modernization research of spleen-stomach dampness-heat syndrome in the future.
