3.0T MRI对桥池段面、听神经的显示及听神经血管压迫的诊断 被引量:1

摘要 目的:探讨3.0TMRI在显示桥池段面、听神经解剖结构的优势及在血管压迫桥池段听神经中的诊断效能。方法:回顾本院初诊听神经血管压迫综合征并行3.0T和1.5T磁共振内耳道检查的病例各32例进行对照分析,由两位神经系统影像学专家辨认桥池段耳蜗神经、前庭上神经、前庭下神经、面神经解剖走行以及判断是否存在血管压迫或接触听神经,使用SPSS软件进行统计学分析。结果:两组均清晰显示桥池段面、听神经结构及走行,表现为条束状低信号,边界清楚,周围见高信号脑脊液环绕;使用3.0T磁共振扫描组能清晰分辨的蜗神经、前庭上/下神经以及听神经血管接触/压迫的部位数比1.5T磁共振扫描组多,其中比较前庭上/下神经显示差别有统计学意义(P<0.05)。结论:3.0TMRI对桥池段蜗神经分支及神经血管压迫的显示较1.5T更具优势,是听神经血管压迫综合征术前的首选检查。 Objective:To investigate the advantages of 3.0 TMRI in displaying the anatomical structure of facial nerve and auditory nerve,and the diagnostic efficacy of auditory nerve vascular compression in cerebellopontine angle cistern.Methods:Retrospective of 32 cases of primary diagnosed auditory neurovascular compression syndrome with 3.0 T and 1.5 T magnetic resonance examination of the inner ear canal in our hospital,then conducted for control analysis.Two neuroimaging experts were employed to identify the anatomical course of the cochlear nerve,superior vestibular nerve,inferior vestibular nerve and facial nerve in cerebellopontine angle cistern,and to determine whether there was vascular compression or contact with the auditory nerve.SPSS software was used for statistical analysis.Results:Facial nerve and auditory nerve structure and course were clearly displayed in both groups,showing bundle of low signal and clear boundary,which surrounded by high signal cerebrospinal fluid.The number of contact/compression sites of cochlear nerve,superior vestibular/inferior vestibular nerve and auditory nerve vessels that could be clearly identified in the 3.0 T MRI scanning group was more than that in the 1.5 T MRI scanning group,and the superior vestibular/inferior vestibular nerve between two groups showed statistical significance(P<0.05).Conclusion:3.0 TMRI is more advantageous than 1.5 T in the detection of cochlear nerve branches and neurovascular compression in cerebellopontine angle cistern,and it is the first choice for preoperative examination of auditory neurovascular compression syndrome.
