吞咽造影联合吞咽康复治疗对脑卒中后吞咽障碍患者康复效果的影响

Effect of swallowing radiography combined with swallowing rehabilitation therapy on the rehabilitation of patients with dysphagia after stroke

目的探讨吞咽造影联合吞咽康复治疗对脑卒中后吞咽障碍患者康复效果的影响。方法选取2019年4月至2020年10月抚州市第一人民医院收治的76例脑卒中后吞咽障碍患者作为研究对象,按随机数字表法分为对照组(38例)和观察组(38例)。对照组采取吞咽康复治疗,观察组在吞咽康复治疗基础上联合X线动态碘海醇吞咽造影,干预时间均为2周。比较两组的吞咽功能、误吸程度及并发症总发生率。结果干预后,观察组的吞咽功能分级优于对照组,且恢复率高于对照组,差异均有统计学意义(P<0.05)。干预前,两组的误吸程度评分比较,差异无统计学意义(P>0.05);干预后,观察组的误吸程度评分和并发症总发生率低于对照组,差异均有统计学意义(P<0.05)。结论吞咽造影联合吞咽康复治疗能改善脑卒中后吞咽障碍患者康复效果,降低并发症的发生,值得临床推广。

Objective To investigate the effect of swallowing radiography combined with swallowing rehabilitation therapy on the rehabilitation of patients with dysphagia after stroke.Methods A total of 76 patients with dysphagia after stroke admitted to Fuzhou First People′s Hospital from April 2019 to October 2020 were selected as the research subjects.The patients were divided into the control group(n=38)and the observation group(n=38)according to random number table method.Patients in the control group were provided with swallowing rehabilitation therapy,and patients in the observation group were combined with X-ray dynamic iohexol swallowing angiography on the basis of swallowing rehabilitation therapy.The intervention time lasted two weeks.The swallowing function,the degree of aspiration and the overall incidence of complications were compared between the two groups.Results After the intervention,the swallowing function classification of the observation group was better than that of the control group,and the recovery rate was higher than that of the control group,with statistical differences(P<0.05).Before the intervention,there was no significant difference in the aspiration degree score of the two groups(P>0.05).After the intervention,the aspiration degree score and the overall incidence of complications in the observation group were lower than those in the control group,with statistical differences(P<0.05).Conclusion Swallowing radiography combined with swallowing rehabilitation therapy can improve the rehabilitation effect of patients with dysphagia after stroke,reduce complications,which is worthy of clinical promotion.
