新生儿截石体位搁脚架的研究与应用

Research and application of lithotomy position footrest in neonates

目的探讨截石体位搁脚架在新生儿手术中的应用效果。方法选取2017年1月至2018年12月在赣南医学院第一附属医院行肛肠类手术治疗的35例新生儿作为研究对象,并采用随机数字表法分为常规组(18例)和研究组(17例)。在手术过程中,常规组采用下肢悬吊法,研究组采用截石位搁脚架,比较两组的使用体位摆放时长、术野范围、新生儿皮肤完整性、手术医生满意度、家属满意度。结果研究组的使用体位摆放时长短于常规组,差异有统计学意义(P<0.05);研究组的术野范围清晰程度高于常规组,差异有统计学意义(P<0.05);研究组的皮肤压红、勒痕、末梢循环障碍总发生率低于常规组,差异有统计学意义(P<0.05);研究组的手术医生满意度、家属满意度均高于常规组,差异有统计学意义(P<0.05)。结论在新生儿手术中采用截石体位搁脚架,能缩短使用体位摆放时间,提高术野清晰度,保证皮肤的完整性,且手术医师和新生儿家属的满意度高。

Objective To explore the application effect of lithotomy position footrest in neonatal surgeries.Methods A total of 35 neonates who underwent anorectal surgery in the First Affiliated Hospital of Gannan Medical University from January 2017 to December 2018 were selected as the research objectives,and divided into the routine group(18 cases)and the study group(17 cases)according to the random number table method.During the surgery,the routine group were given lower limb suspension,and the study group were given lithotomy position footrest.The length of posture,scope of surgical field,skin integrity,and degrees of satisfaction of surgeons and family members were compared between the two groups.Results The length of posture in the study group was shorter than that in the routine group,and the difference was statistically significant(P<0.05).The scope of surgical field in the study group was clearer than that in the routine group,and the difference was statistically significant(P<0.05).The total incidences of skin redness,strangulation,and peripheral circulatory disorders in the study group were lower than those in the conventional group,and the differences were statistically significant(P<0.05).The degrees of satisfaction of surgeons and family members in the study group were higher than those in the conventional group,and the differences were statistically significant(P<0.05).Conclusion The application of lithotomy position footrest can shorten the length of posture,improve the clarity of surgical field,ensure the integrity of skin in neonatal surgeries,with high degrees of satisfaction of surgeons and family members.
