口腔按摩联合婴儿体位评估工具下鸟巢体位在低出生体质量早产儿中的应用 被引量:6

摘要 目的探讨SandraFucile口腔按摩联合婴儿体位评估工具(the infant position assessment tool,IPAT)指导下鸟巢体位对低出生体质量早产儿吸吮功能、神经发育的影响。方法按照随机数字表将郑州大学第三附属医院2017年7月至2019年7月收治的127例低出生体质量早产儿分组,对照组63例给予IPAT指导下鸟巢体位支持,观察组64例在对照组基础上增加SandraFucile口腔按摩。观察两组患儿新生儿重症监护病房(neonatal intensive care unit,NICU)相关质控指标、吸吮功能、喂养状态和神经功能发育情况。结果观察组干预后胃管留置(29.98±5.69)d、全肠内营养(26.56±4.23)d、呼吸机使用时间(635.23±10.21)h及NICU住院天数(2.36±0.21)个月较对照组(35.24±6.21)d、(30.29±2.57)d、(729.35±14.45)h、(2.85±0.33)个月短(P<0.05);观察组干预7 d(37.86±6.42)分、10 d(52.42±6.42)分、14 d(72.58±8.48)分吸吮功能评分均较对照组(34.26±5.86)分、(47.33±5.75)分、(65.45±7.52)分高(P<0.05);观察组开始经口喂养(32.15±0.63)周和完全经口喂养(33.26±0.82)周胎龄较对照组(33.24±0.84)周、(35.89±1.57)周小、开始经口喂养(5.32±1.58)mL/min和完全经口喂养效率(13.23±3.24)mL/min较对照组(4.78±1.21)mL/min、(9.56±2.87)mL/min快(P<0.05);观察组3、6月龄Infanib评估正常比例优于对照组(P<0.05)。结论SandraFucile口腔按摩SandraFucile口腔按摩联合IPAT指导下鸟巢体位可通过刺激NICU低出生体质量早产儿口腔运动,提高吸吮功能,缩短全肠内营养,呼吸机使用时间,改善喂养状态,促进神经功能发育。
