黄连解毒汤对肛周脓肿根治术后患者创面愈合及肛门直肠压力的影响

Effect of Huanglian Jiedu Decoction on Wound Healing and Anorectal Pressure after Radical Operation of Perianal Abscess

目的研究黄连解毒汤对肛周脓肿根治术后患者创面愈合及肛门直肠压力的影响。方法此次的100例研究对象均为收治的肛周脓肿患者中回顾性选取,选例时间控制在2018年1月—2020年1月,依据治疗方式的不同将其分为对照组(50例)和试验组(50例)。对照组接受高锰酸钾溶液熏洗治疗,试验组则接受黄连解毒汤水煎液熏洗治疗,2组均连续治疗1周。比较2组治疗后的临床疗效、创面愈合情况以及治疗前、治疗后7 d、治疗后15 d的肛门直肠压力。结果治疗后经比较发现,试验组和对照组总有效率分别为88.00%和70.00%,试验组与对照组比较,明显处于较高水平;试验组创面腐肉脱落时间、新生上皮出现时间以及创口愈合时间均明显短于对照组;与治疗前比,治疗后2组肛管最大收缩压、肛管静息压以及直肠最大耐受阈值均呈现先降低后升高的趋势,与治疗后7 d比,2组均呈现升高的趋势,且试验组均高于对照组(均P<0.05)。结论黄连解毒汤可缓解肛周脓肿根治术后患者的临床症状,可促进创面愈合,同时提高肛门直肠压力,加快术后恢复进程,治疗效果显著。

Objective To study the effect of Huanglian Jiedu decoction on wound healing and anorectal pressure in patients after radical resection of perianal abscess.Methods The 100 study subjects were selected retrospectively from the patients with perianal abscess admitted to our hospital.The selection time was controlled from January 2018 to January 2020.They were divided into a control group according to different treatment methods(n=50)and an experimental group(n=50).Patients in the control group received potassium permanganate solution fumigation treatment,and patients in the experimental group received Huanglian Jiedu decoction fumigation treatment.Patients in both groups received continuous treatment for 1 week.The clinical efficacy,wound healing,and anorectal pressure before treatment,7 days after treatment,and 15 days after treatment were compared between the two groups of patients.Results After treatment,it was found that the total effective rates of the experimental group and the control group were 88.00%and 70.00%,respectively.Compared with the control group,the experimental group was obviously at a higher level.The time of sloughing of wounds,the appearance of new epithelium,and the healing time of wounds in the experimental group were significantly shorter than those in the control group.Compared with before treatment,the maximum systolic pressure of the anal canal,the resting pressure of the anal canal and the maximum tolerance threshold of the rectum of the two groups of patients after treatment decreased first and then increased.Compared with the 7 days after treatment,both groups showed an increase.The experimental group was higher than the control group(all P<0.05).Conclusion Huanglian Jiedu decoction can relieve the clinical symptoms of patients after radical resection of perianal abscess,promote wound healing,and at the same time increase anorectal pressure,speed up the postoperative recovery process,and has significantly therapeutic effect.
