他汀类药物个体化用药研究进展

Research progress in the individualized use of statins

他汀类药物是临床使用最为广泛的降脂药之一,除了降脂作用以外,还有其他药理作用,其临床疗效具有明显的个体差异,部分人群会出现肌肉毒性,严重者出现横纹肌溶解症。造成药物个体差异的主要因素是基因多态性,主要体现在有机阴离子转运多肽1B1(OATP1B1)基因、ApoE基因、细胞色素P450酶家族基因、ABCB1基因、HMG-CoA还原酶基因、ABCG2基因的多态性对他汀类药物的药效、不良反应、用药剂量等有不同的影响,故临床用药前对患者进行基因检测至关重要。本文总结上述基因多态性对他汀类药物影响的研究进展和指南,分析他汀类个体化用药方案,为他汀类药物个体化用药提供参考。

Statins are one of the most widely used lipid-lowering drugs in clinical practice.In addition to lipid-lowering effects,statins have other pharmacological effects,and their clinical efficacy has obvious individual differences.Muscular toxicity and rhabdomyolysis may occur in some people.The main factor of individual differences caused by drugs is gene polymorphisms,mainly reflects in the effect of polymorphism of organic anion-transporting peptide 1B1(OATP1B1)gene,ApoE gene,cytochromatin P450 enzyme family gene,ABCB1 gene,HMG-CoA reductase gene,and ABCG2 on statins,and the adverse reactions and dosage,so before clinical application it is important to make genetic testing.In this paper,the progress and guidelines of the study on the influence of the above gene polymorphism on statins were summarized,and the individualized regimen of statins was analyzed to provide reference for the individualized use of statins.
