标准造口护理联合集束化护理对结直肠癌术后肠造口患者的疗效观察

Efficacy of standardized stoma nursing combined with bundles of care in patients with colorectal cancer undergoing postoperative enterostomy

目的探讨标准造口护理流程联合集束化护理策略应用于结直肠癌术后肠造口患者的临床效果。方法选取2018年6月至2020年6月间西安大兴医院收治的114例结直肠癌术后患者,采用随机信封抽签原则分为集束组和对照组,每组57例。对照组患者采用标准造口护理流程干预,集束组患者在对照组基础上联合集束化护理策略干预,比较两组患者的临床疗效。结果两组患者手术时间和术中出血量比较,差异无统计学意义(P>0.05)。集束组患者术后下床时间、术后排气时间和术后住院时间均少于对照组,差异均有统计学意义(均P<0.05)。集束组患者护理期间造口并发症发生率为3.5%,低于对照组的19.3%,差异有统计学意义(均P<0.05)。护理后,两组患者自我护理能力评分均高于护理前,且集束组高于对照组,差异均有统计学意义(均P<0.05)。护理后,集束组患者护理满意度为100.0%,高于对照组的86.0%,差异有统计学意义(均P<0.05)。结论标准造口护理流程联合集束化护理策略应用于结直肠癌术后肠造口患者,可减少患者并发症发生,提高护理满意度与自我护理能力,促进康复。

Objective To explore and observe the efficacy of standardized stoma nursing combined with bundles of care in patients with colonic stoma after surgery for colorectal cancer.Methods From June2018 to June 2020,114 patients with colorectal cancer who were admitted to Xi’an Daxing Hospital were selected.They were divided into a bundles of care group and a control group with 57 patients in each groups using principle of random envelope drawing.The bundles of care group was given bundles of care based on standardized stoma nursing and the control group was only given standardized stoma nursing.Clinical efficacy was compared between the two groups.Results There was no significant difference in duration of operation and intraoperative blood loss between the two groups(P>0.05).The time to out-of-bed actives,time to postoperative exhaust and length of postoperative hospital stay were less in the bundles of care group than in the control group(all P<0.05).The incidence of stomal complications was 3.5%in the bundles of care group,which was lower than 19.3%of the control group(P<0.05).The scores of self-care ability were higher in the two groups after nursing than those before nursing(all P<0.05),and the scores were higher in the bundles of care group than in the control group(all P<0.05).The satisfaction was 100.0%in the bundles of care group after nursing,which was higher than 86.0%of the control group(P<0.05).Conclusion The standardized stoma nursing combined with bundles of care can reduce the incidence of complications,promote patient recovery,and improve patient satisfaction and self-care ability in patients with intestinal stoma after surgery for colorectal cancer.
