摘要
目的探讨血清干扰素α(IFN-α)、抗心磷脂抗体(aCL)和抗β2糖蛋白Ⅰ(β2-GPI)抗体与系统性红斑狼疮(SLE)患者血小板减少的相关性。方法选取2018年1月~2019年6月在我院门诊及住院就诊的系统性红斑狼疮患者65例,其中血小板减少SLE患者20例(PLT<100×109/L,A组),无血小板减少组45例(101×109/L~370×109/L,B组),选取同期16例脾功能亢进(HS)患者为疾病对照组(C组),选取同期我院体检中心的健康体检者60人(D组)。酶联免疫吸附(ELISA)法检测四组血清IFN-α水平、抗aCL抗体、抗β2-GPI抗体和抗双链DNA抗体滴度,免疫比浊法检测CRP、补体C3和C4。结果A组SLE患者血清IFN-α水平、aCL和抗β2-GPI总抗体滴度均显著高于其余三组(均P<0.01),B组显著高于C和D组(P<0.05);A组抗dsDNA抗体滴度、CRP浓度和SLEDAI显著高于B组(均P<0.01),补体C3和C4显著低于B组(均P<0.01);血清IFN-α水平与aCL和抗β2-GP1总抗体滴度呈显著正相关(r=0.754,0.814,均P<0.01),IFN-α水平、aCL和抗β2-GPI总抗体滴度均与血小板呈显著负相关(r=0.651,0.717,0.688,均P<0.01)。结论SLE患者体内IFN-α水平、aCL和抗β2-GPⅠ总抗体滴度均与血小板呈负相关,提示IFN-α、aCL和抗β2-GPⅠ抗体与血小板减少有关。
Objective To explore the correlation between serum IFN-α,anti-cardiolipin antibody(ACL),anti-β2-glycoprotein1(β2-gpⅠ)antibody and thrombocytopenia in patients with systemic lupus erythematosus(SLE).Methods Sixty-five SLE patients diagnosed at our hospital from January 2018 to June 2019 were included in the study.The SLE group was further divided into two groups according to thrombocytopenia or not,thrombocytopenia(PLT<100×109/L,group A,20)and without thrombocytopenia(101×109/L-370×109/L,groupB,45).Sixteen patients with hypersplenism in the same period were selected as disease control group(Group C),and sixty healthy persons(Group D)were selected from the physical examination center of our hospital at serum IFN-αlevel,anti-ACL antibody,anti-β2gpⅠand anti-dsDNA antibody titer were detected by ELISA,CRP,complement C3 and C4 were detected by immunoturbidimetry.Results Serum IFN-αlevel,anti-ACL antibody and anti-β2-gpⅠantibody titers in group A were significantly higher than those in the other three groups(P<0.01),and those indexes in group B were significantly higher than groups C and D(P<0.05).The anti-dsDNA antibody titer,CRP concentration and SLEDAI in group A were significantly higher than group B(all P<0.01),and complement C3 and C4 were significantly lower than group B(all P<0.01).Serum IFN-αlevels were significantly positively correlated with ACL and anti-β2gpⅠtotal antibody titers(r=0.754,0.814,all P<0.01).IFN-αlevel,ACL and anti-β2gpⅠtotal antibody titers were significantly negatively correlated with platelets(r=0.651,0.717,0.688,all P<0.01).Conclusion IFN-αlevel,ACL and anti-β2gpⅠtotal antibody titer were negatively correlated with platelet,sugested that IFN-α,ACL and anti-β2gpⅠantibodies are associated with thrombocytopenia.
