PABD对行肝癌根治术患者的炎性因子、免疫指标及预后的影响

Effect of PABD on Inflammatory Factors,Immune Parameters and Prognosis in Patients Underwent Radical Hepatocellular Carcinoma Surgery

目的探讨PABD对行肝癌根治术患者的炎性因子,免疫指标及预后的影响。方法将收治的62例肝癌患者为研究对象(均行肝癌根治术治疗)。根据围术期输血方式的差异分为2组,各31例。研究组采取PABD,而对照组采取异体输血方式,比较2组患者的临床疗效等差异。结果术后2 h,两组患者PT及APTT均长于术前,而FIB均低于术前(P<0.05);术前两组免疫细胞指标无差异(P>0.05),而术后研究组免疫细胞指标较对照组高(P<0.05);术前两组炎性指标无差异(P>0.05),而术后研究组炎性指标较对照组高(P<0.05);术后1年,两组患者的复发率比较无差异(P>0.05)。结论PABD可以降低对于实施肝癌根治性切除患者的机体免疫抑制功能,亦不会对其肿瘤的复发和凝血功能产生显著影响,值得临床推广使用。

Objective To investigate the effects of PABD on inflammatory factors,immune parameters and prognosis in patients undergoing radical hepatocellular carcinoma surgery.Methods 62 patients with hepatocellular carcinoma were selected(all were treated with radical hepatocellular carcinoma).The patients were divided into 2 groups,31 cases in each group,according to the difference of perioperative transfusion methods.The study group adopted PABD,while the control group adopted allogeneic blood transfusion,and the differences in clinical efficacy between the 2 groups were compared.Results At 2 h after surgery,PT and APTT were longer than before surgery in both groups,while FIB was lower than before surgery in both groups(P<0.05);There was no difference in immune cell indexes between the 2 groups before surgery(P>0.05),while immune cell indexes were higher in the study group than in the control group after surgery(P<0.05);There was no difference in inflammatory indexes between the 2 groups before surgery(P>0.05),while inflammatory indexes were higher in the study group than in the control group after surgery(P<0.05);1 year after surgery,there was no difference in the recurrence rate between the two groups(P>0.05).Conclusion PABD can reduce the immunosuppressive function of the body in patients underwent radical resection for hepatocellular carcinoma,and does not significantly affect the tumor recurrence and coagulation function,which is worthy of clinical promotion.
