公立医院高质量发展背景下医疗联合体合作策略研究

Research on the Cooperation Strategy of Medical Institutions in Medical Association in the Context of High-Quality Development of Public Hospitals

目的:以博弈理论为基础,通过对医联体内核心医院和成员医院进行效益矩阵分析,明确合作中最优发展策略,提出医联体在组建中的帕累托改进措施。方法:以公立医院高质量发展为背景,从博弈论的视角以医联体建设中核心医院和成员医院是否真正合作作为博弈点,探讨合作中的决策行为,建立博弈模型,分析收益矩阵。结果:政府的奖惩机制和不合作的"不正当"收益对核心医院和成员医院影响最大。结论:从政府、核心医院和成员医院的角度,提出加强政府奖惩机制、构建双赢合作机制、建立紧密型城市医疗集团、成员医院提升技术能力等措施,对组建策略进行帕累托改进。

Objective:Based on game theory,the optimal development strategy in the cooperation is clarified by analyzing the ben?efit matrix of core hospital and member hospitals in the medical alliance.Based on the literature,Pareto improvement measures in the formation of medical consortia are proposed.Methods:In the context of high-quality development of public hospitals,whether core hospital and member hospitals truly cooperate in the construction of medical consortia is taken as a game point from the perspective of game theory to explore the decision-making behavior in cooperation,establish a game model,and analyze the benefit matrix.Results:The government’s reward and punishment mechanism and the"improper"benefits of non-cooperation have the greatest influence on the core hospitals and member hospitals.Conclusion:From the perspectives of government,core hospitals and member hospitals,it proposes measures to strengthen the government reward and punishment mechanism,build a win-win cooperation mechanism,establish a close urban medical group,and improve the technical capacity of member hospitals to improve the formation strategy of Pareto.
