团队管理式延续性护理在2型糖尿病患者中的应用价值

The Application Value of Team Management Continuation Care in Patients with Type 2 Diabetes

目的探讨团队管理式延续性护理在2型糖尿病患者中的应用价值。方法按照抛硬币法将2018年12月至2020年12月我院收治的2型糖尿病患者102例分成干预组与参照组,各51例,参照组给予常规的延续性护理,干预组实施团队管理式延续性护理。比较干预前后两组血糖、自我管理能力、情绪状况差异,并记录与统计患者的再入院率、护理满意度。结果干预前,两组血糖(空腹血糖、餐后2 h血糖)、自我管理能力(饮食管理、运动管理、血糖管理、足部监测)、情绪状况比较均无明显差异(P>0.05);干预6个月后,两组空腹血糖、餐后2 h血糖、焦虑及抑郁人数均显著降低,饮食管理、运动管理、血糖管理、足部监测评分均显著升高,且干预组上述指标均显著优于参照组(P<0.05)。结论团队管理式延续性护理可以提高2型糖尿病患者的临床治疗效果,且自我管理能力与满意度均得以提高,焦虑抑郁情绪得以改善。

Objective To explore the application value of team management team management continuation care in patients with type 2 diabetes.Methods According to the coin toss method,102 patients with type 2 diabetes admitted to our hospital from December 2018 to December 2020 were divided into an intervention group and a reference group with 51 cases each.The reference group was given routine continuation nursing,and the intervention group implemented a team management style.The differences in blood glucose,self-management ability,and emotional status between the two groups before and after the intervention were compared,and the readmission rate and nursing satisfaction of the patients were recorded and counted.Results Before the intervention,there was no significant difference in blood glucose(fasting blood glucose,2 hours postprandial blood glucose),self-management ability(diet management,exercise management,blood glucose management,foot monitoring),and emotional status between the two groups(P>0.05);intervention After 6 months,the number of fasting blood glucose,2 hours postprandial blood glucose,anxiety and depression in the two groups were significantly reduced,and the scores of diet management,exercise management,blood glucose management,and foot monitoring were significantly increased,and the above indicators of the intervention group were significantly better.In the reference group(P<0.05).Conclusion The team-managed continuation nursing may improve the clinical treatment effect of patients with type 2 diabetes,and the self-management ability and satisfaction can be improved,and anxiety and depression can be improved.
