益气养血助产汤联合理气行血针法在低位及出口难产中的临床价值

Clinical value of Yiqi Yangxue midwifery Decoction combined with Liqi Xingxue acupuncture in low and outlet dystocia

目的探讨益气养血助产汤联合理气行血针法在低位及出口难产中的应用价值。方法采用随机数字表法将秦皇岛市妇幼保健院收治的82例低位及出口难产产妇分为对照组与治疗组,每组41例。对照组行低位或出口产钳助产术,治疗组实施中医针药结合(益气养血助产汤+理气行血针法)助产技术。对比两组产程时间、母体损伤率、产后出血量、围产儿损伤率、新生儿阿氏(Apgar)评分。结果(1)产程时间:两组第一产程、第二产程比较差异无统计学意义(P>0.05),治疗组第三产程短于对照组(P<0.05);(2)母体损伤率:治疗组母体损伤率12.20%低于对照组39.02%(P<0.05);(3)母体产后出血量:治疗组母体产后2 h与产后24 h出血量少于对照组(P<0.05);(4)围产儿损伤率:治疗组围产儿损伤率4.88%低于对照组34.15%(P<0.05);(5)新生儿Apgar评分:治疗组新生儿出生1 min、出生5 min及出生10 minApgar评分高于对照组(P<0.05)。结论益气养血助产汤联合理气行血针法可促进低位及出口难产产妇产程进展,降低母婴损伤发生率,减少产后出血,且有利于改善母婴健康状态。

Objective To explore the application value of Yiqi Yangxue midwifery decoction combined with Liqi Xingx⁃ue acupuncture in low and outlet dystocia.Methods 82 cases with low and outlet dystocia admitted to Qinhuangdao maternal and Child Health Hospital were randomly divided into control group(n=41)and treatment group(n=41)The control group was given low or export forceps midwifery,while the treatment group was given traditional Chinese medi⁃cine combined with acupuncture(Yiqi Yangxue midwifery decoction+Liqi Xingxue acupuncture).The duration of la⁃bor,maternal injury rate,postpartum hemorrhage,perinatal injury rate and neonatal Apgar score were compared be⁃tween the two groups.Results(1)Labor time:there was no significant difference in the first and second stages of labor between the two groups(P>0.05),and the third stage of labor in the treatment group was shorter than that in the control group(P<0.05);(2)Maternal injury rate:the maternal injury rate of the treatment group was 12.20%,which was lower than 39.02%of the control group(P<0.05);(3)Maternal postpartum hemorrhage:the maternal postpartum 2 h and 24 h hemorrhage in the treatment group was less than that in the control group(P<0.05);(4)Perinatal injury rate:the perinatal injury rate of the treatment group was 4.88%,which was lower than 34.15%of the control group(P<0.05);(5)Neonatal Apgar score:The Apgar scores at 1 min,5 min and 10 min after birth in the treatment group were higher than those in the control group(P<0.05).Conclusion Yiqi Yangxue midwifery Decoction combined with Liqi Xingxue acupuncture can promote the progress of low and export dystocia,cut down the incidence of maternal and in⁃fant injury,reduce postpartum hemorrhage,and improve the health of mothers and infants.
