高分辨率MRI联合微小RNA-135a-5p、微小RNA-373对宫颈癌的诊断价值研究

Diagnostic value of high-resolution MRI combined with serum miR-135a-5p and miR-373 expression in cervical cancer

目的探讨高分辨率MRI联合微小RNA-135a-5p(miR-135a-5p)、微小RNA-373(miR-373)对宫颈癌的诊断价值。方法选取2018年9月至2020年9月浙江省台州医院进行宫颈病理检查的患者132例,根据病理诊断结果将其分为宫颈癌89例(恶性组)与宫颈癌前病变43例(良性组),所有患者均给予高分辨率MRI检查,记录检查参数并分析诊断价值,采用实时荧光定量(RT-PCR)检测两组患者血清miR-135a-5p、miR-373表达水平,分析高分辨率MRI、miR-135a-5p、miR-373表达水平与宫颈癌的关系,以及单项检测和3项联合检测的诊断价值。结果与良性组相比,恶性组miR-135a-5p、miR-373表达水平较高(均P<0.05)。阳性高危型HPV、鳞癌、低分化、有淋巴结转移、TNM分期Ⅲ~Ⅳ期、有淋巴血管间隙浸润及深浸润宫颈癌患者miR-135a-5p、miR-373表达水平均高于阴性高危型HPV、腺癌、高与中分化、无淋巴结转移、TNM分期Ⅰ~Ⅱ期、无淋巴血管间隙浸润及无浸润与浅浸润宫颈癌患者,差异均有统计学意义(均P<0.05)。ROC曲线分析显示,与高分辨率MRI、miR-135a-5p、miR-373单项检测相比,3项联合检测对宫颈癌的诊断效能较高(P<0.05)。结论高分辨率MRI可准确显示宫颈癌的征象,miR-135a-5p、miR-373在宫颈癌患者血清中呈异常高表达且与患者临床特征有关,3项联合检测对宫颈癌的诊断价值较高。

Objective To investigate the diagnostic value of high-resolution MRI combined with serummicroRNA-135a-5p(miR-135a-5p)and microRNA-373(miR-373)expression for cervical cancer.Methods Eighty nine patients with cervical cancer(malignant group)and 43 patients with cervical precancerous lesions(benign group)pathologically diagnosed in Taizhou hospital from September 2018 to September 2020 were enrolled in the study.All patients underwent high-resolution MRI examination,serum miR-135a-5p and miR-373 expression was detected by real-time fluorescence quantitative(RT-PCR)in the two groups,the relationship between high-resolution MRI,miR-135a-5p,miR-373 expression and cervical cancer was analyzed.Results Compared with the benign group,the expression of miR-135a-5p and miR-373 was higher in the malignant group(P<0.05).Compared with patients with negative high-risk HPV,adenocarcinoma,high and moderately differentiated degree,no lymph node metastasis,stageⅠ-Ⅱ,no lymphatic vascular space invasion and non-invasive,superficially invasive cervical cancer,the expression of miR-135a-5p and miR-373 was significantly higher in patients with positive high-risk HPV,squamous cell carcinoma,low differentiation degree,lymph node metastasis,stageⅢ-Ⅳ,lymphatic vascular space invasion and deep invasive cervical cancer(all P<0.05).Compared with high-resolution MRI,miR-135a-5p and miR-373 expression alone,ROC curve analysis showed that the high-resolution MRI combined with miR-135a-5p and miR-373 had a higher diagnostic value for cervical cancer(P<0.05).Conclusion High-resolution MRI can accurately display the signs of cervical cancer.miR-135a-5p and miR-373 are highly expressed in the serum of patients with cervical cancer and are related to the clinical characteristics of the patients.The combined detection of the three indicators has a high predictive value for cervical cancer.
