散结消瘿膏联合泼尼松片治疗复发性亚急性甲状腺炎的疗效观察

Observation on the Curative Effect of Sanjie Xiaoying Ointment Combined with Prednisone Tablets in the Treatment of Recurrent Subacute Thyroiditis

目的探讨散结消瘿膏联合泼尼松片治疗复发性亚急性甲状腺炎(subacute thyroiditis,SAT)的临床疗效及安全性。方法选取2019年1月—2020年6月期间于河南中医药大学第一附属医院就诊的60例复发性SAT患者,随机分为对照组和治疗组各30例。对照组口服泼尼松片,治疗组在此基础上给予散结消瘿膏外敷,两组均治疗12周。治疗后对两组患者临床疗效、中医症状积分、超声检查结果、甲状腺功能指标[游离三碘甲状腺原氨酸(free triiodothyronine,FT3)、游离甲状腺素(free thyroxine,FT4)、促甲状腺素(thyroid stimulating hormone,TSH)]及血沉(erythrocyte sedimentation rate,ESR)水平、并发症发生情况进行分析和比较。结果治疗组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后治疗组中医症状积分与对照组比较明显降低,血流信号恢复时间、肿大消退时间与异常回声消退时间比对照组明显缩短,差异有统计学意义(P<0.05)。治疗后治疗组FT3、FT4、ESR水平与对照组比较明显降低,TSH水平与对照组比较明显升高,差异有统计学意义(P<0.05)。治疗组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论散结消瘿膏外敷联合泼尼松片治疗复发性SAT的效果显著,能明显改善患者的临床症状,调节甲状腺功能和ESR水平,且并发症少。

Objective To investigate the effectiveness and safety of Sanjie Xiaoying ointment combined with prednisone tablets in the treatment of recurrent subacute thyroiditis(SAT).Methods A total of 60 patients with recurrent SAT who were treated in the first affiliated hospital of Henan university of traditional Chinese medicine from January 2019 to June 2020 were selected and divided into a control group and a treatment group,30 cases each.The control group was given oral prednisone tablets,and on this basis,the treatment group was given external application of Sanjie Xiaoying Ointment.After 12 weeks of treatment,the Clinical efficacy,TCM symptom scores,ultrasound examination results,thyroid function indexes[free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH)]and erythrocyte sedimentation rate(ESR)levels for the two groups of patients and complications were analyzed and compared.Results The total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the TCM symptom score of the treatment group was significantly lower than that of the control group.The blood flow signal recovery time,swelling subsidence time and abnormal echo subsiding time were significantly shorter than those of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of FT3,FT4,and ESR in the treatment group were significantly lower than those in the control group,and the TSH levels were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Sanjie Xiaoying ointment combined with prednisone tablets has a remarkable effect on the treatment of recurrent SAT.It can obviously improve the clinical symptoms,regulate thyroid function and ESR level,and has few complications.
