推割刀联合独活寄生汤加减治疗拇指狭窄性腱鞘炎的临床观察

Clinical Observation on Treatment of Stenotic Tenosynovitis of Thumb with Push Cutter Combined with Duhuo Jisheng Decoction

目的评价推割刀联合独活寄生汤加减对拇指狭窄性腱鞘炎患者的临床疗效。方法选取2018年9月—2019年9月期间在北京中医医院顺义中医院就诊的拇指狭窄性腱鞘炎患者60例,随机分为对照组和试验组,各30例。对照组采用单纯推割刀治疗,试验组采用推割刀联合独活寄生汤加减治疗,治疗后评价两组患者掌指关节疼痛评分、手部感觉缺失评分、中医证候评分及临床疗效。结果两组患者治疗后,掌指关节疼痛、手部感觉缺失积分及中医证候评分与治疗前比较,差异均有统计学意义(P<0.05),且试验组治疗后各积分变化均优于对照组,两组比较,差异有统计学意义(P<0.05)。试验组总有效率为100%,对照组为75.86%,两组比较,差异有统计学意义(P<0.05)。结论推割刀联合独活寄生汤加减对拇指狭窄性腱鞘炎患者疗效显著,优于单纯推割刀治疗。

Objective To evaluate the clinical effect of pushing and cutting knife combined with Duhuo Jisheng Decoction on patients with thumb stenotic tenosynovitis.Methods 60 patients with stenotic tenosynovitis of thumb treated in Shunyi Hospital of Beijing Traditional Chinese medicine hospital from September 2018 to September 2019 were randomly divided into control group and experimental group,with 30 cases in each group.The control group was treated with simple push knife,and the experimental group was treated with push knife combined with Duhuo Jisheng Decoction.After treatment,the metacarpophalangeal joint pain score,hand sensory loss score,TCM syndrome score and clinical efficacy of the two groups were evaluated.Results After the two groups of patients were treated,there were statistically significant differences between the test group and the control group for metacarpophalangeal joint pain,hand sensation loss,and TCM syndrome scores compared with those before treatment(P<0.05).The score changes were better than those of the control group,and the difference between the two groups was statistically significant(P<0.05).The total effective rate of the experimental group was 100%,and that of the control group was 75.86%.The difference between the two groups was statistically significant(P<0.05).Conclusion The curative effect of pushing and cutting knife combined with Duhuo Jisheng Decoction on patients with thumb stenotic tenosynovitis is significant,which is better than that of pushing and cutting knife alone.
