床旁超声在PICU重症肺炎患儿诊断中的应用价值 被引量:2

摘要 目的探讨床旁超声在儿童重症监护室(PICU)重症肺炎患儿诊断中的应用价值。方法选取2020年1月至2020年12月常德市第一人民医院PICU收治的重症肺炎患儿60例,根据重症肺炎诊断标准,60例均确诊为重症肺炎,均进行了床旁超声检查,观察超声图像结果并记录。结果60例患儿通过床旁超声检查,检出58例重症肺炎,检出率为96.7%,其超声图像表现为:可见B线58例占比100%,胸膜线异常45例占比77.6%,肺实变伴支气管充气征19例占比32.8%,胸腔积液5例占比8.6%,肺滑动征消失3例占比5.2%。结论床旁超声在PICU重症肺炎患儿诊断中准确率高,可作为PICU重症肺炎患儿的诊断工具,应加大力度推动其在重症肺炎诊断中的应用。 【Objective】To explore the application value of bedside ultrasound in the diagnosis of severe pneumonia in Pediatric Intensive Care Unit(PICU).【Methods】According to the diagnostic criteria for severe pneumonia,a total of 60 children with severe pneumonia admitted to PICU of the First People's Hospital of Changde from January 2020 to December 2020 were selected for bedside ultrasound examination,and the ultrasound image results were observed and recorded.【Results】A total of 58 cases in 60 children with severe pneumonia were detected through bedside ultrasound examination,with a detection rate of 96.7%.The ultrasound image indicated that 58 cases of B line accounted for 100%,and 45 cases of abnormal pleural line accounted for 77.6%.Nineteen cases of pulmonary consolidation with bronchial inflation sign accounted for 32.8%,5 cases of pleural effusion accounted for 8.6%,and 3 cases of atelectasis accounted for 5.2%.【Conclusion】Bedside ultrasound has a high accuracy rate in the diagnosis of PICU children with severe pneumonia.It can be used as a diagnostic tool in PICU children with severe pneumonia.More efforts should be made to promote its application in the diagnosis of severe pneumonia.
