抗生素骨水泥型假体旷置治疗髋关节置换术后假体周围感染的效果分析

Effect of antibiotic bone cement prosthesis in treatment for periprosthesis infection after hip replacement

目的探讨患者在髋关节置换术后出现假体周围感染后采用抗生素骨水泥型假体旷置治疗的应用效果。方法选取2019年2月至2020年9月河南省洛阳正骨医院收治的30例髋关节置换术后出现假体周围感染情况的患者临床资料,采用抗生素骨水泥型假体旷置疗法对患者进行治疗并观察记录相关数据。测评患者术前2周和术后2个月髋关节功能(Harris评分)、疼痛程度评分(VAS)及红细胞沉降率(ESR)、C反应蛋白(CRP)水平情况。结果患者术后2周拆线后,其中29名患者创口恢复情况佳,未出现红肿热痛等症状。患者术后2个月Harris评分较术前2周升高,但术后2个月VAS评分较术前2周降低(P<0.05)。患者术后2个月CRP、ESR水平较术前2周降低(P<0.05)。结论对于出现髋关节置换术后假体周围感染的患者给予抗生素骨水泥型假体旷置疗法,其具有显著的优越性,有利于积极控制感染,改善患者术后髋关节功能,提高临床疗效。

【Objective】To investigate the application effect of antibiotic bone cement prosthesis in the treatment of periprosthesis infection after hip arthroplasty.【Methods】Thirty patients with periprosthetic infection after hip replacement were recruited from February 2019 to September 2020 in our department.Antibiotic bone cement type prosthesis was used for the treatment of patients included in the study and relevant data were observed and recorded.The patients'hip function(Harris score),pain severity(VAS),erythrocyte sepultation rate(ESR),and C-reactive protein(CRP)were evaluated 2 weeks before and 2 months after surgery.【Results】The suture was removed 2 weeks after operation,and 29 of them had good wound healing without symptoms of redness,swelling,heat and pain.Compared with preoperation,Harris score of patients 2 months after operation was significantly increased,and VAS score,CRP,ESR were also significantly decreased.The difference was statistically significant(all P<005).【Conclusion】For patients with periprosthesis infection after hip arthroplasty,the administration of antibiotic bone cement prosthesis open placement therapy has significant advantages,which is beneficial to actively control infection,improve postoperative hip function of patients,and improve clinical efficacy.
