血常规中血细胞计数与骨密度,骨代谢产物结果的相关性分析

Analysis of the Correlation Between Blood Cell Count in Blood Routine Tests and Bone Mineral Density and Bone Metabolites

摘要 目的研究老年人群入院后血常规中的血细胞计数与腰椎、股骨颈两部位的骨密度以及血清β-胶原特殊序列、总Ⅰ型胶原氨基端延长肽、25-羟基总维生素D、钙、磷浓度之间的相关性。方法回顾性分析2016年11月—2021年2月住院病人90例,记录患者腰、髋两部位双能X线骨密度值检测结果报告中的T值、骨代谢产物检测结果(包括β-胶原特殊序列、总Ⅰ型胶原氨基端延长肽、25-羟基总维生素D)以及血常规检查结果(包括红细胞计数、白细胞计数、淋巴细胞计数、中性粒细胞计数、单核细胞计数、嗜酸性粒细胞计数、嗜碱性粒细胞计数以及血小板计数)、血清电解质检查结果(包括血钙、血磷),分别进行各变量与骨密度的相关性研究。结果腰椎的T值与淋巴细胞计数具有相关性(P<0.05),总Ⅰ型胶原氨基端延长肽的含量与血小板计数有相关性(P<0.05),血钙表现出与红细胞计数以及淋巴细胞计数显著的相关性(P<0.01),血磷也表现出与红细胞计数以及淋巴细胞计数之间的相关性(P<0.05),同时血磷也与血小板计数有一定的相关性(P<0.05)。结论血常规中红细胞计数、淋巴细胞计数可以较好地反映血钙和血磷的变化情况,而血小板计数在一定程度上与总Ⅰ型胶原氨基端延长肽呈正相关。可以通过关注这些血常规检查中反映出来的结果,做到对骨质疏松症的早期发现,以求早期预防,提高患者的生活质量。 Objective To study the correlation between blood cell count and bone mineral density of lumbar vertebrae and femoral neck,special sequence of serumβ-collagen,total typeⅠcollagen N-terminal extension peptide,25-hydroxy total vitamin D,calcium and phosphorus in elderly people after admission.Methods 90 inpatients from November 2016 to February 2021 were retrospective analyzed,the T value in the report of dualenergy X-ray bone mineral density of each person's waist and hip,the detection results of bone metabolites(including special sequence ofβ-collagen,total typeⅠcollagen N-terminal extension peptide,25-hydroxy total vitamin D)and blood routine examination results(including red blood cell count,white blood cell count,lymphocyte count,neutrophil count,monocyte count,eosinophil count,basophil count and platelet count),and serum electrolyte test results(including blood calcium,blood phosphorus)were recorded.The correlation between each variable and bone mineral density was studied.Results The T value of lumbar vertebrae was correlated with lymphocyte count(P<0.05),and the content of total typeⅠcollagen N-terminal extension peptide was correlated with platelet count(P<0.05).Serum calcium was significantly correlated with red blood cell count and lymphocyte count(P<0.01),and blood phosphorus was also correlated with red blood cell count and lymphocyte count(P<0.05).At the same time,there was a certain correlation between blood phosphorus and platelet count(P<0.05).Conclusion In blood routine,red blood cell count and lymphocyte count can better reflect the changes of blood calcium and phosphorus,while platelet count is positively correlated with total typeⅠcollagen N-terminal extension peptide to some extent.By paying attention to the results reflected in these blood routine tests,we can achieve the early detection of osteoporosis in order to prevent early and improve the quality of life of patients.
