经尿道电切术联合盐酸吉西他滨灌注治疗浅表性膀胱肿瘤患者的临床效果

Clinical Effect of Transurethral Resection Combined with Gemcitabine Hydrochloride Infusion in the Treatment of Superficial Bladder Tumor

目的分析经尿道电切术联合盐酸吉西他滨灌注治疗浅表性膀胱肿瘤患者的临床效果。方法选取70例2015年3月至2018年5月期间在我院接受治疗的浅表性膀胱肿瘤患者,随机分为对照组和研究组,均为35例。对照组采用经尿道电切术联合丝裂霉素灌注治疗,研究组采用经尿道电切术联合盐酸吉西他滨灌注治疗,比较两组的手术时间、术后留置导尿管时间、住院时间、术后12个月内并发症发生率,治疗后1年和2年的复发率。结果研究组术后12个月内的并发症发生率(2.86%)低于对照组(17.14%),差异有统计学意义(χ^(2)=3.9683,P<0.05)。对照组在治疗后1年和2年的复发率分别为5.71%和20.00%;研究组分别为2.85%和5.71%,在治疗后2年的复发率研究组显著低于对照组,差异有统计学意义(χ^(2)=3.2188,P<0.05)。结论经尿道电切术联合盐酸吉西他滨灌注治疗浅表性膀胱肿瘤患者可减少灌注不良反应,同时可有效降低复发率。

Objective To analyze the clinical effect of transurethral resection combined with gemcitabine hydrochloride perfusion in the treatment of superficial bladder tumor.Methods A total of 70 patients with superficial bladder tumor treated in our hospital from March 2015 to may 2018 were randomly divided into control group and study group,35 cases in each group.The control group was treated with transurethral resection combined with mitomycin perfusion,and the study group was treated with transurethral resection combined with gemcitabine hydrochloride perfusion.The operation time,postoperative indwelling catheter time,hospital stay,complication rate within 12 months after operation,recurrence rate of 1 year and 2 years after treatment were compared between the two groups.Results The incidence of postoperative complications in the study group(2.86%)was lower than that in the control group(17.14%),the difference was statistically significant(χ^(2)=3.9683,P<0.05).The recurrence rate of the control group was 5.71%and 20.00%in the first year and the second year after treatment.The recurrence rate of the study group was significantly lower than that of the control group,the difference was statistically significant(χ^(2)=3.2188,P<0.05).Conclusion Transurethral resection combined with gemcitabine hydrochloride perfusion in the treatment of superficial bladder tumor can reduce the adverse reaction of perfusion,and can effectively reduce the recurrence rate.
