摘要
目的分析血清微小RNA(micorRNA,miR)-132、miR-155水平在心力衰竭患者中的表达以及其与心肌重构的关系。方法选取慢性心力衰竭(CHF)患者184例为研究组,另选取健康者200例为对照组。比较2组血清miR-132、miR-155水平以及心肌重构指标。采用ROC曲线分析。结果研究组血清miR-155表达、左心室质量指数(LVMI)、左心室后壁厚度(LVPW)、左心室舒张末期内径(LVEDD)、左心房内径(LAD)明显高于对照组,miR-132表达、左心室重构指数(LVRI)明显低于对照组(P<0.01)。miR-155水平与LVMI呈正相关,miR-132水平与LVMI呈负相关(P<0.01)。NYHA分级、miR-155、miR-132、LVMI、LVRI、LVPW、LVEDD、LAD是影响CHF患者预后的独立危险因素(P<0.05,P<0.01)。血清miR-132、miR-155预测预后不良的ROC曲线下面积分别为0.848(95%CI:0.791~0.918)、0.837(95%CI:0.774~0.904)。结论血清miR-132表达水平与心肌重构呈正相关,miR-155表达水平与心肌重构呈负相关,两者均是CHF患者预后的独立危险因素。
Objective To analyze the expression of serum miR-132 and miR-155 in HF patients and its relationship with myocardial remodeling(MR).Methods One hundred and eighty CHF patients served as a study group and 200 healthy people served as a control group.The expression of serum miR-132 and miR-155 and related indexes of MR were compared between the two groups.The value of serum miR-132 and miR-155 for predicting the outcome in CHF patients was analyzed by ROC curve analysis.Results The miR-155 expression level and LVMI were significantly higher,the LVPW was significantly thicker,the LVEDD and LAD were significantly longer while the miR-132 expression level and LVRI were significantly lower in study group than in control group(P<0.01).Pearson correlation analysis showed that the serum level of miR-155 was positively related while that of miR-132 was negatively related with the LVMI in CHF patients(P<0.01).The NYHA grading,serum miR-155 and miR-132 expression levels,LVMI,LVRI,LVPW,LVEDD and LAD were independent risk factors for the outcomes in CHF patients(P<0.05,P<0.01).ROC curve analysis displayed that the AUC of serum miR-132 and miR-155 level for predicting the poor outcomes in CHF patients was 0.848(95%CI:0.791-0.918)and 0.837(95%CI:0.774-0.904)respectively.Conclusion The expression level of serum miR-132 is positively related while that of serum miR-155 is negatively related with the MR.Serum miR-132 and miR-155 levels are independent risk factors for the outcomes in CHF patients.
