摘要
目的探讨血清血管生成素样蛋白2(ANGPTL2)和外周血单核细胞NF-κB mRNA表达预测急性脑梗死(ACI)患者脑水肿的价值。方法选取本院收治的ACI患者240例,根据头颅CT检查分为脑水肿组58例和对照组182例。ELISA法测血清ANGPTL2水平,RT-PCR法测NF-κB mRNA表达。采用多因素logistic回归分析脑水肿的影响因素,绘制ROC曲线分析两者预测脑水肿的价值。结果脑水肿组血清ANGPTL2和NF-κB mRNA表达高于对照组(P=0.000)。多因素logistic回归分析显示,血清ANGPTL2和NF-κB mRNA表达是脑水肿影响因素(P<0.05)。血清ANGPTL2和NF-κB mRNA表达预测脑水肿的ROC曲线下面积0.887、0.823,均有预测价值。结论血清ANGPTL2及单核细胞NF-κB mRNA表达是ACI患者脑水肿影响因素,可早期预测脑水肿。
Objective To study the value of serum ANGPTL2 level and NF-κB mRNA expression in PBMC for predicting cerebral edema in ACI patients.Methods Two hundred and forty ACI patients admitted to our hospital were divided into cerebral edema group(n=58)and control group(n=182)according to their head CT findings.Their serum ANGPTL2 level was measured by ELISA and their NF-κB mRNA expression was detected by RT-PCR.The influencing factors of cerebral edema were analyzed by multivariate logistic regression analysis.The value of serum ANGPTL2 level and NF-κB mRNA expression in PBMC for predicting cerebral edema in ACI patients was analyzed by ROC curve analysis.Results The serum ANGPTL2 level and NF-κB mRNA expression level were significantly higher in cerebral edema group than in control group(P=0.000).Multivariate logistic regression analysis showed that serum ANGPTL2 level and NF-κB mRNA expression level were influence factors of cerebral edema in ACI patients(P<0.05).The AUC of serum ANGPTL2 level and NF-κB mRNA expression level for predicting cerebral edema in ACI patients was 0.887(95%CI:0.770-1.000)and 0.823(95%CI:0.679-0.968)respectively.Conclusion Serum ANGPTL2 level and NF-κB mRNA expression level are the influencing factors of cerebral edema and can thus predict early cerebral edema in ACI patients.
