摘要
目的探讨低张水充盈法联合多排螺旋CT三期增强扫描对胃癌分期的应用价值。方法选择我院2018年3月至2020年6月收治的200例经胃镜病理确诊为胃癌患者作为研究对象,根据随机抽签分组原则分为观察组100例、对照组100例,分别采取低张水充盈法联合CT三期增强方式、常规CT扫描方式,分析影像学表现并且与术后病理结果对照,对比两组各项指标。结果参照AJCC第8版TNM分期标准,观察组总检出率97.00%,高于对照组患者检出率90.00%(P<0.05)。观察组T分期准确率74.00%(T1期8例、T2期9例、T3期7例、T4期50例),高于对照组69.00%,差异有统计学意义(P<0.05)。N分期和M分期两组间符合率相近。结论低张水充盈法安全易行,联合CT三期增强扫描对胃癌治疗前准确分期具有较高的应用价值。
Objective To investigate the value of hypotonic filling combined with three-phase enhanced multi-slice spiral CT in the staging of gastric cancer.Methods A total of 200 patients with gastric cancer diagnosed by gastroscopy pathology in our hospital from March 2018 to June 2020 were selected as the research objects.According to the principle of random lot,they were divided into observation group and control group.Hypotonic water filling method combined with three-phase CT enhancement and conventional CT scanning were adopted respectively to analyze the imaging manifestations and compare with the postoperative pathological results.The indexes of the two groups were compared.Results According to the TNM staging standard of AJCC version 8,the total detection rate in the observation group was 97.00%,which was higher than 90.00%in the control group(P<0.05).The accuracy rate of T stage in the observation group was 74.00%(8 cases in T1 stage,9 cases in T2 stage,7 cases in T3 stage and 50 cases in T4 stage),which was significantly higher than 69.00%in the control group,the difference was statistically significant(P<0.05).The coincidence rate between N stage and M stage was similar.Conclusion Hypotonic filling method is safe and easy,and combined with three-stage enhanced CT scanning has high application value for accurate staging of gastric cancer before treatment.
