流程优化4A模型视角下智慧门诊和应急管理的探索实践

Exploration and Practice of Intelligent Outpatient and Emergency Management from the Perspective of Process Improvement 4A Model

目的:以流程优化4A模型为理论框架,从流程的评估、分析、改进、实施4个方面对江苏省人民医院门诊部的发热预检分诊和防控前沿化措施进行探索,寻求形成可控性、有效性、常态化防控策略。方法:通过实地调研、访谈法、头脑风暴法、数据分析法,对突发公共事件下江苏省人民医院门诊部就医流程存在问题及多次改进情况和现状进行分析、评估。结果:基于国家防控政策,疫情常态化下,江苏省人民医院门诊部根据实际需求多次流程优化后,建立诊前-诊中-诊后全方位防控。结合互联网+智慧医疗,线上、线下相结合布局疫情防控,线上充分利用互联网医院;线下引入智慧闸机、检测点迁移、服务中心关口前移、自助机区前摆等形成一体化预检分诊,契合舆论引导和宣传教育将防控前沿化,定期进行核酸阳性患者的隔离预案和演练。结论:就目前来看疫情防控的前沿化预防在我国相对较少,其流程优化十分必要,具有督促卫生应急的作用,阻断传播途径,也可快速分流、高效检测、有效避免感染,从而保障百姓的生命安全,防止快速扩散。

Objective To explore the advance fever triage and prevention and control measures in the outpatient department of Jiangsu Provincial People’s Hospital from four aspects of process evaluation, analysis, improvement and implementation, and form controllable, effective and normalized prevention and control strategies. Methods Through field research, interview, brainstorm and data analysis, the problems in the process of medical treatment in the outpatient department of Jiangsu Provincial People’s Hospital under public emergencies and its improvement were analyzed and evaluated. Results Based on national prevention and control policy and the normalized epidemic situation, a comprehensive prevention and control system of pre-diagnosis, in-treatment and post-diagnosis was established in the outpatient department of Jiangsu Provincial People’s Hospital, which combined the Internet with smart medical treatment, and integrated online with offline prevention and control measures. Conclusion The frontier prevention of epidemic prevention and control is relatively less in China, and its process optimization is very necessary, which has the role of urging health emergency, blocking route of transmission, achieving fast diversion and efficient detection, and effectively avoiding infection, so as to protect the people’s life safety and prevent its rapid spread.
