糖尿病体检者糖脂代谢指标与高尿酸血症之间的关联性分析 被引量:1

摘要 目的探究糖尿病体检者糖脂代谢指标与高尿酸血症之间的关联性。方法回顾性选取2019年5月—2021年5月于该单位体检的70例2型糖尿病(T2DM)体检者的临床资料,整理比较其临床资料,70例T2DM体检者中有21例合并高尿酸血症(HUA),设为HUA组,其余设为对照组,分析T2DM体检者糖脂代谢与HUA的相关性。结果HUA组体检者血尿酸(SUA)水平为(485.45±59.48)μmol/L,明显高于对照组的(278.65±45.41)μmol/L,差异有统计学意义(t=15.869,P<0.05);HUA组体检者空腹血糖(FPG)、餐后2 h血糖(2 hPG)、空腹胰岛素(FINS)及胰岛素抵抗指数(HOMA-IR)水平均明显高于对照组,差异有统计学意义(P<0.05);HUA组体检者总胆固醇(TC)、三酰甘油(TG)均明显高于对照组,差异有统计学意义(P<0.05),但高密度脂蛋白(HDL)明显低于对照组,差异有统计学意义(P<0.05),两组体检者低密度脂蛋白(LDL)水平比较差异无统计学意义(P>0.05);体检者SUA水平与糖代谢指标FPG、2 hPG、FINS及HOMA-IR呈显著的正相关(P<0.05);与脂代谢指标TC、TG呈显著的正相关(P<0.05),与HDL呈显著的负相关(P<0.05)。结论T2DM合并HUA体检者的糖脂代谢存在明显异常,且体检者SUA水平与糖脂代谢存在显著相关性,密切关注二者联系,可为T2DM和HUA临床诊治提供指导。 Objective To explore the correlation between the glucose and lipid metabolism indexes and hyperuricemia in diabetic patients.Methods Retrospectively selected the clinical data of 70 patients with type 2 diabetes(T2DM)who had a physical examination in the unit from May 2019 to May 2021,and sorted and compared their clinical data.Among the 70 patients with T2DM,21 cases had Hyperuri cemia(HUA).Set as the HUA group,and the rest as the control group,and analyze the correlation between glucose and lipid metabolism and HUA in T2DM subjects.Results The serum uric acid(SUA)level of the physical examination subjects in the HUA group was(485.45±59.48)μmol/L,which was significantly higher than that of the control group(278.65±45.41)μmol/L,and the difference was statistically significant(t=15.869,P<0.05);the fasting blood glucose(FPG)of the physical examination subjects in the HUA group,2 h postprandial blood glucose(2 hPG),fasting insulin(FINS)and insulin resistance index(HOMA-IR)levels were significantly higher than those of the control group,and the difference was statistically significant(P<0.05);the total cholesterol(TC)and triglycerides of the physical examination subjects in the HUA group(TG)were significantly higher than the control group,and the difference was statistically significant(P<0.05),but the high-density lipoprotein(HDL)was significantly lower than the control group,and the difference was statistically significant(P<0.05).There was no statistical difference in the levels of low-density lipoprotein(LDL)between the two groups(P>0.05);the level of SUA in the physical examination was significantly positively correlated with glucose metabolism indexes FPG,2 hPG,FINS and HOMA-IR(P<0.05);it was significantly positively correlated with lipid metabolism indexes TC and TG(P<0.05),which was significantly negatively correlated with HDL(P<0.05).Conclusion The glucose and lipid metabolism of patients with T2DM combined with HUA physical examination is obviously abnormal,and there is a significant correlation between the level of SUA and glucose and lipid metabolism in physical examination subjects.Paying close attention to the relationship between the two can provide guidance for the clinical diagnosis and treatment of T2DM and HUA.
