鼠李糖联合深度水解蛋白配方对2岁以下牛奶蛋白过敏症患儿生长发育的影响 被引量:6

摘要 目的探讨鼠李糖联合深度水解蛋白配方对2岁以下牛奶蛋白过敏症(CMPA)患儿生长发育的影响。方法选取2019年2月-2020年5月该院收治的2岁以下CMPA患儿96例,按照随机数字法分为蛋白配方组和联合治疗组,各48例。蛋白配方组给予患儿深度水解蛋白配方进行治疗,联合治疗组给予患儿鼠李糖联合深度水解蛋白配方进行治疗。使用液相色谱质谱联用法对患儿嗜酸性粒细胞计数(EOS)、特异性牛奶蛋白(IgE)水平检测;对比两组患儿生长发育指标(体质量、身高及头围);使用全自动生化分析仪对患儿三酰甘油、总胆固醇、前白蛋白、中性粒细胞及血小板水平检测对比;对比两组治疗有效率。结果治疗后两组患者EOS、IgE水平有所下降,且与蛋白配方组相比,联合治疗组EOS、IgE水平较低(t=15.010,t=78.780,均P<0.05);治疗后两组患者相比,体质量、身高及头围有所增加,且与蛋白配方相比,联合治疗组体质量、身高及头围有所增加(t=2.725,t=2.477,t=2.715,均P<0.05);治疗后两组患者三酰甘油、总胆固醇及前白蛋白水平上升,与蛋白配方相比,联合治疗组三酰甘油、总胆固醇及前白蛋白水平有所上升(t=2.661,t=5.947,t=39.320,均P<0.05);治疗后两组患儿中性粒细胞有所减少,血小板水平下降,且与蛋白配方组相比,联合治疗组中性粒细胞较低,血小板水平较低(t=9.418,t=3.501,均P<0.05)。结论使用鼠李糖联合深度水解蛋白配方对2岁以下CMPA患儿进行治疗,能够缓解患儿的过敏症状,改善患儿营养状况,有利于患儿的生长发育,具有较好的治疗效果。 Objective To investigate the effects of a combined deep hydrolyzed protein formula of rhamnose on the growth and development of children under 2 years of age with cow’s milk protein allergy(CMPA).Methods Ninety six children under 2 years old with cow’s milk protein allergy(CMPA) treated in our hospital from February 2019 to may 2020 were selected and divided into protein formula group and combination therapy group according to the random number method, with 48 cases each.The treatment with deep hydrolyzed protein formula was given to children in the protein formula group, and the treatment with rhamnose combined with deep hydrolyzed protein formula was given to children in the combined treatment group.Eosinophil count(EOS) and specific cow’s milk protein(IGE) levels were measured using liquid chromatography mass spectrometry(LC-MS);measures of growth and development [body mass, height, head circumference] were compared between the two groups;levels of triglycerides, total cholesterol, prealbumin, neutrophils, and platelets were compared between the two groupsRate.Results After treatment, EOS and IgE levels in both groups decreased, and the EOS and IgE levels in the combined treatment group were lower than those in the protein formula group(t=15.010, t=78.780,P<0.05);After treatment, the body weight, height and head circumference of patients in the two groups were increased, and the body weight, height and head circumference of patients in the combined treatment group were increased compared with those in the protein formula group(t=2.725,t=2.477, t=2.715, P<0.05);After treatment, the levels of triglyceride, total cholesterol and prealbumin in 2 groups increased, and those in the combined treatment group increased compared with the protein formula(t=2.661,t= 5.947,t=39.320, P<0.05);After treatment, neutrophils and platelet levels decreased in both groups, and compared with the protein formula group, neutrophils and platelet levels were lower in the combined treatment group(t=9.418,t=3.501,P<0.05).Conclusion Treatment of children under 2 years of age with CMPA using rhamnose combined with deep hydrolyzed protein formula was able to relieve their allergic symptoms, improve the nutritional status of the children, benefit the growth and development of the children, and have a better therapeutic effect.
