经脐单孔腹腔镜与腹腔镜手术治疗胆囊良性疾病的疗效比较

Clinical efficacy of benign disease of gallbladder:Single port laparoscopic cholecystectomy vs laparoscopic cholecystectomy

目的回顾性对比分析经脐单孔腹腔镜与腹腔镜手术治疗胆囊良性疾病的临床疗效。方法回顾性分析2018年1月-2020年1月在合肥市第二人民医院普外科治疗的胆囊良性疾病的患者110例,其中选择经脐单孔腹腔镜胆囊切除术(SILC)的患者共65例,常规腹腔镜胆囊切除术(CLC)的患者45例,随访至术后1月时间,比较两组的患者相关参数方面的差异。结果以性别、年龄、良性疾病类型,ASA评分、两组间没有显著差异,SILC组和CLC组比较,术后阿片类止痛药使用率、疼痛评分及美容效果评分,有显著性差异,在手术时间、术中出血量、术后放置腹腔引流管情况、术后炎症指标及对肝功能影响,两组间无显著性差异,术后两组患者转氨酶均有升高。结论SILC对胆囊良性疾病患者一种安全、有效的方法,可以有效的减轻了患者术后疼痛,改善美容效果,提高了手术质量。

Objective To retrospectively analyze the clinical efficacy of SILC and CLC in the treatment of benign disease of gallbladder.Methods A retrospective analysis of 110 elderly patients with benign disease of gallbladder,treated at the Second People’s Hospital of Hefei from January 2018 to January 2020.65 patients were selected for SILC and 45 were patients with CLC.The differences in parameters related to the patients in the SILC group and the CLC group were compared.Results There were no significant differences between the two groups based on gender,age,ASA score,BMI,and diseases.Compared with the CLC group,postoperative opioid analgesics usage rate,pain score and cosmetic effect score,were significantly reduced,There was no significant difference between the two groups during the operation time,intraoperative blood loss,postoperative placement of abdominal drainage tube and the liver function,Patients with elevated transaminases after surgery in both groups.Conclusion SILC is a safe and effective method for patients with benign gallbladder disease,which can effectively reduce postoperative pain,improve cosmetic effects,,and improve the quality of surgery.
